The basic medical insurance system is one of the hotpots in the current health policy research,and the medical insurance system of economic risk control effect of the basic medical insurance system is the core content of the study.The role of reducing household catastrophic health expenditure is an important indicator of the economic risk control effect.China's three basic medical insurance systems were born at the end of the last century and the beginning of this century,which were Government-led establishment of the medical market system,and it is to effectively protect the interests of residents,to avoid the individual and family suffering huge economic losses due to the disease,to protect the basic health needs of residents.With the rising health care costs,whether the basic medical security system can effectively reduce the catastrophic health expenditure of the residents?Whether our three basic health care systems vary in economic protect effect?Therefore,it is necessary to explore and analyze these issues in depth.The paper aims to measure the disease economic risk of Chinese families with the elderly suffered from chronic diseases,and to estimate the influencing factors,and to specify the different economic protection effects of the three health care insurance systems and two kinds of health care insurance systems based on the China Health and Retirement Longitudinal Study.We use logistic multivariate regression analysis and multiple linear regression analysis.The results show the incidence and intensity of catastrophic health expenditure are higher in Chinese elderly households,and are higher in chronic disease families than non-chronic disease ones both in rural and urban areas.We find that the main factors which affect the households'catastrophic health economic expenditure include:marital status,whether to get financial help,or to own members over 65,or to have members who suffer two or more chronic diseases.The protection effect of NCMS(New Rural Cooperative Insurance)in reducing catastrophic health expenditure is significantly lower than that of medical insurance for urban employees and urban residents.There was no significant difference in the protective effects of basic medical insurance for urban employees and basic medical insurance for urban residents on reducing catastrophic health expenditure.The economic protection of mandatory health care insurance is significantly better than voluntary insurance.We suggest that the government should also attach importance to the fairness of the medical security level while focusing on the coverage of insurance,and should pay attention to the elderly with chronic diseases especially the vulnerable groups in rural areas.The funding levels need to be improved in rural areas,and the reimbursement of outpatient and inpatient Medical Expenses for Patients with Chronic Diseases should be improved at the same time.
