Effect Of Rate-adaptive Dual-chamber Pacing On Cardiac Function And Quality Of Life In The Elderly With Sick Sinus Symptom

Objective: Analyses the effect of rate-adaptive dual-chamber pacing on cardiacfunction and quality of life in the elderly with Sick Sinus Symptom.Methods:62patients who come from Yijishan Hospital Cardiology with sick sinussyndrome were enrolled and divided into rate responsive pacing group (32cases)and non-rate responsive pacing group(30case).According to the postoperative1month,6monthsfollow-up program to observe pacemaker system and patients clinical comprehensivereaction. Cardiac function was evaluated in terms of echocardiography parameters(LAEDD,LVEDD,LVEF),6MWT and BNP during follow-up period. the quality of lifewere analyses by means of SF-36.The correlation between above mentioned parameterswere analyzed.Results:1The changes appear on the inner diameter of the patient’s heart(LAEDD, LAEDD)between DDDR group and DDD group.1.1Postoperative1month[LAEDD(40.65±4.42VS41.67±3.76，P>0.05),LVEDD(48.66±6.76VS47.60±4.97,P>0.05)]1.2Postoperative6months[LAEDD(41.38±4.37VS41.10±3.57，P>0.05),LVEDD(47.78±6.19VS48.33±5.08,P>0.05)]2The changes appear on Cardiac function(LVEF、6-MWT、BNP) between DDDR group and DDD group.2.1Postoperative1month [LVEF(62.19±7.39VS60.17±7.40,P>0.05),6-MWT(350.94±76.02VS314.73±60.24,P<0.05,BNP（77.69±24.63VS83.67±25.55，P>0.05）)]2.2Postoperative6months[LVEF(66.38±5.75VS62.80±5.07,P<0.05),6-MWT(432.94±50.14VS385.20±57.83,P<0.05,BNP（64.84±21.27VS77.03±23.55，P<0.05）)]3The changes appear on the quality of life(General Health) between DDDR group andDDD group.3.1Postoperative1month [General Health（65.40±9.20VS58.93±10.43，P<0.05）]3.2Postoperative6months [General Health（71.75±6.79VS63.93±8.96，P<0.01）]Conclusion:1.Dual chamber pacemaker can not reduce the inner diameter of the patient’s heart;2.Rate-adaptive pacemaker can’t improve heart function obviously in the short term,as thelonger the time of implantation, cardiac function can improve significantly, especially interms of exercise tolerance;3.Rate-adaptive pacemaker can significantly improve the quality of life of patients.
