Correlation Between High-frequency Ultrasonography And Functional Outcome In Post-arthroscopic Rotator Cuff Repair Patients And Predictor Of Rotator Cuff Repair Outcome Research

[Purpose] The correlation between high-frequency ultrasonic findings and functional outcome after arthroscopic repair for rotator cuff tear patients is discussed while the relevant factors that influence postoperative recovery are explored.[Methods] To analyze on30cases of followed patients getting rotator cuff repair in our department owing to "rotator cuff tear" during the period from December,2011to February,2014.We assessed functional outcomes respectively by the Fudan University Shoulder Scoring system(FDSS), the UCLA scoring system and the ASES rating scale.The anatomical recovery was assessed by measuring the supraspinatus tendon thickness(STT) and the acromiohumeral distance(AHD)of both shoulders(healthy side noted as STT’ and AHD’)under high-frequency ultrasound.To compare the FDSS scale with other two shoulder functional scores respectively and to calculate Pearson Correlation Coefficient on evaluate validity.We respectively calculated occupation ratio(equals STT/AHD) and tendon thickness ratio(equals STT/STT’)through mentioned quantitative indices got by ultrasonic measurement.After that,both the unary linear regression and locally weighted linear regression analysis will be done respectively between these indices,age,supraspinatus tear size,postoperative duration,surgical procedure,involved side and the FDSS score.And the main predictors of patient’s functional outcome will be explored through multiple linear regression analysis.[Results]2~28months follow-up period for30cases,with the mean period of12.0±8.2months.The grading range of the FDSS score is from43to99,with the mean score73.0±17.7.The grading range of the UCLA score is from17to35,with the mean score27.6±6.1.The grading range of the ASES score is from33to100,with the mean score71.1±22.2.Pearson Correlation Coefficient of linear regression for the FDSS score and the UCLA score is0.936(p<0.001),and0.966(p<0.001) for the FDSS score and the ASES score.The correlation of unary linear regression of tendon thickness ratio(equals STT/STT’),age,supraspinatus tear size,postoperative duration and the FDSS score is strong (the P value is relatively0.002,0.003,0.005and0.003) while the correlation of locally weighted linear regression is also strong (the P value is relatively0.001,0.003,0.001and <0.001).Meanwhile,the correlation of unary linear regression of occupation ratio(equals STT/AHD),surgical procedure,ill shoulders and the FDSS score is weak (the P value is relatively0.111,0.568and0.837) as the correlation of locally weighted linear regression also being weak (the P value is relatively0.068,0.516and0.837).Through the analysis of multiple linear regression,it is shown that the outcomes of rotator cuff repair is age-related,changing with tendon thickness ratio over postoperative time.[Conclusions] The FDSS scoring system is of a favorable validity evaluation,and it can be used as the evaluation for the shoulder joint function of Chinese people. The functional outcome of post-arthroscopic rotator cuff repair patients is irrelevant to occupation ratio,surgical procedure and involved side,which is related to age and supraspinatus tear size,changing with tendon thickness ratio over postoperative time.The tendon thickness ratio by high-frequency ultrasound may be one of recommendable indices for follow-up observation at patients after rotator cuff repair.
