Purpose:The purpose of our study was to evaluate clinical results of arthroscopic rotator cuff repairs with concomitant manipulation in patients with preoperative shoulder stiffness.Methods:Among patients who underwent arthroscopic rotator cuff repairs between April2009and April2012,30patients with shoulder stiffness were enrolled for our study.We evaluate the therapeutic effects through observation and comparison the changes of VAS,ROM,muscle strength and shoulder scores(CMS,UCLA,SST) of patients before and after the therapy.Results:Thirty patients with complete follow-up documentation for24months were included in the outcome analyses.Pain:the mean VAS,at rest was2.06and0.55(P=0.036),respectively before the operation and at the last follow-up.the mean VAS during motion was7.30and0.95(P=0.012),respectively before the operation and at the last follow-up.Range of Motion:Before the operation and at the last follow-up,forward flexion was73.45°and169.33°(P=0.004),abduction was74.50°and170.43°(P=0.002),external rotation at the side was33.45°and50.62°(P=0.045),whereas internal rotation to the back was greater trochanter of femur level and T10level (P<0.001).Patients had gradual recovery of forward flexion,abduction,external rotation at the side and internal rotation to the posterior.The ROM of contralateral shoulders was almost identical to that in the shoulders after surgery at the last follow-up.Muscle Strength:Before the operation and at the last follow-up,forward flexion was3.25kg and7.05kg(P=0.004),external rotation at the side was5.35kg and7.23kg(P=0.039),whereas internal rotation at the side was6.27kg and8.34kg(0.017).Clinical Assessment:CMS was93.2and90.9(P=0.003),UCLA was34.3and33.0(P=0.002),SST was34.3and33.0(P=0.003),respectively before the operation and at the last follow-up.Conclusions:The results of investigation demonstrated that overall satisfactory results were achieved in selected patients who underwent one-stage arthroscopic repair with concomitant manipulation in rotator cuff tears with shoulder stiffness.The results also showed that pain,ROM,muscle strength and function scores improved significantly than those of before the operation.
