The Curative Effect Analysis Of Huge Hepatocellular Carcinoma By Surgical Resection

Background: The surgical resection of huge hepatocellular carcinoma (HCC) isdisputed for a long time. This study was designed to introduce our clinical experienceof precise hepatectomy for huge HCC and assessed the safety and outcomes of precisehepatectomy for huge HCC.Methods: A total of89precise hepatectomies for the patients with huge HCC wereanalysed retrospectively from December2007to March2014. The outcomes werecompared with106patients with HCC which tumor diameter between5.0cm and10.0cm in the Department of Hepatobiliary Surgery of PLA hospital．Prognostic factorsfor long-term survival were evaluated by univariate and multivariate analyses.Results: The1-,3-,5-year overall survival rates of huge HCC after precisehepatectomy were78.4%，42.1%and12.5%. Overall survival and disease-free survivalin huge HCC group and HCC (5.0cm-10.0cm) group were similar (P=0.062,P=0.0.065). Univariate and multivariate analysis revealed that the HBV infection, thenumbers of tumour, complications，surgical resection margin，remnant liver volume andthe ratio of remnant liver volume and standard liver volume were independentprognostic factors for overall survival (P=0.001, P=0.035, P=0.022, P=0.005, P=0.037，P=0.011). Univariate and multivariate analysis also showed that the types of resection,the numbers of tumour, complications，surgical resection margin，blood loss,remnantliver volume and the ratio of remnant liver volume and standard liver volume wereindependent prognostic factors for recurrence (P=0.027, P=0.031, P=0.001, P=0.000,P=0.028, P=0.016).Conclusions: Precise hepatectomy can be performed safely for huge HCC with a low mortality and favorable survival outcomes.
