Objective:To analyze the clinical and pathologic factors influencing outcome of huge hepatocellular carcinoma (HCC) in patients underwent hepatectomy.Methods:A sample of124patients with huge hepatocellular carcinoma underwent hepatectomy and confirmed as HCC in histopathology from January2004to December2010in our hospital. Univariate analysis and multivariate analysis were performed using statistical software (SPSS16.0for Windows) to identify independent prognostic factors.Results:The cumulative1-,3-,5-year survival rates of124patients were65.1%,35.8%and25.1%, respectively. The mean survival and the median survival was34.7and26.0months, respectively. In the65cases of curative resection, the1-,3-and5-year disease-free survival rates were40.2%,19.3%and7.1%, respectively. The mean disease-free survival and the median disease-free survival was18.6and9.0months, respectively.Univariate analysis showed that HBsAg, tumor capsule, liver cirrhosis, vascular invasion, tumor rupture, intrahepatic-metastasis,curative resection, BCLC stage, significantly affect the postoperative survival(P<0.05, table14). The Cox multivariate analysis indicated that HBsAg, liver cirrhosis, intrahepatic-metastasis, curative resection were independent prognostic factor(P<0.05, table15).Conclusions:HBsAg, tumor capsule, liver cirrhosis, vascular invasion, tumor rupture, intrahepatic-metastasis, curative resection, BCLC stage are the clinical and Pathologic factors influencing outcome of huge HCC in patients undergoing hepatectomy; HBsAg, liver cirrhosis, intrahepatic-metastasis, curative resection are independent prognostic factor; Improving curative resection rate of huge HCC significantly extend the survival time of patients.
