ObjectiveBy comparing the expression of the difference protein and clinical pathologicalcharacteristics between a recurrence/metastasis group and non B recurrence/metastasisgroup.These patients underwent radical resection. In order to construct a mathematic modelpredicting the recurrence/metastasis of HHCC at early time.Methods1. A total of70patients with primary HHCC larger than10cm were recruited fromJanuary2000to December2013from the1st hospital of-----. These patients underwentradical resection and the clinicopathology was retrospectively analyzed. The specimens weredivided into3groups according to clinical follow-up recurrence/metastasis or not (follow-uptime are24months) after receiving HHCC resection:a recurrence/metastasis time≤6monthsgroup (n=39) and a non-recurrence/metastasis>6months group (n=31).Univariable andmultivariable Logistic regression analyses were performed and a mathematic modelpredicting the recurrence/metastasis of HHCC at different time points was constructed.2.The techniques of tissue microarray and immunohistochemical staining wereemployed to examine the expression profiles of AMACR、APOA2、IQGAP2、OPA1、S100A12、S100A6in specimens (both carcinoma tissues and peri-carcinoma tissues). Therelationship between their expression, clinicalpathological features and early postoperativerecurrence and metastasis of huge hepatocellular carcinoma were analyzed by univariate andmultivariate logistic regression analysis and to establish the prediction equation of earlyrecurrence and metastasis.and to analysis various index of the survival curve.Results1.By univariate analysis, age,tumor capsule,endoscopic vascular invasion, IQGAP-2、OPA-1、S100A12、S100A6are risk factors for early postoperative recurrence and metastasis ofhuge hepatocellular carcinoma(P<0.05); multivariate logistic regression analysis revealedthat age, tumor capsule, IQGAP2、S100A6are independent risk factors of huge hepatocellularcarcinoma in patients with early postoperative recurrence and metastasis(P<0.05). The equation for the prediction of recurrence/metastasis within6months wasY=-13.936+2.213*age-2.878*tumor capsule+2.743*IQGAP+1.738*S100A6.2.by univariate analysis of6kinds of proteins indicating that IQGAP-2expression may beassociated with tumor capsule(P=0.039)and AMACR expression may be associated withmacroscopic tumor thrombi(P=0.032)。3.by Kaplan-Meier analysis of the relationship between each protein expression and patientsurvival rates and discoveried: OPA-1expression is associated with patient survival (LogRank [Mantel-Cox] P=0.003); high expression median survival time (31months) is13-month higher than the low expression median survival time (18months), and the restproteins expression and patient survival time was no significant correlation.Conclusions1. Age,cancer capsule,IQGAP-2,S100A6proteins may be the important evidence for theearly recurrence and metastasis of the HHCC; Y=-13.936+2.213*age-2.878*cancer capsule+2.743*IQGAP+1.738*S100A6to a certain extent have served as a kind of predictive effecton. the early recurrence and metastasis of the HHCC in6months.2.OPA1protein expression correlated with survival time of patients with hugehepatocellular carcinoma may be a new molecular biology reference target to predictpostoperative survival time of the HHCC patients.
