Objective: Observation of Huangqi Guizhi Wuwu Decoction combined conventionalWestern medicine therapy for the treatment of acute myocardial infarction after PCIreperfusion injury in clinical efficacy, and to explore the mechanism of its efficacy.Methods: Randomized controlled trials,58cases diagnosed for the ST-segmentelevation acute myocardial infarction undergoing primary PCI,(TIMI3) after reperfusioninjury patients were randomly divided into2groups: the control group using conventionalWestern medicine treatment test group on the basis of conventional western medicinetreatment plus Astragalus Gui Zhi Wu decoction decoction, treatment for12weeks.Observe the two treatment methods of ECG, arrhythmia, heart function, angina, majoradverse cardiac events, TCM syndrome curative effect, and so the impact of acomprehensive evaluation of its efficacy, and serum hs-CRP, lipoprotein (a), D dimer,endothelin, NT-BNP levels to explore its possible mechanism.Results:1.Test group ECG the sum of ST elevation drop ratio is significantly better than thatin the control group (P <0.05), and the experimental group more effectively reducearrhythmia (P <0.05).2.12weeks after the test group, echocardiography left ventricular ejection fraction,wall motion score than the control group (P <0.05). Test of patients NT-BNP levelsdecreased (P <0.05) than before treatment. After treatment between the two groups, the testgroup decreased significantly (P <0.05).3. Angina improvement of the experimental group was significantly better than the control group (P <0.05). The low incidence of the test group of major adverse cardiacevents (P <0.05).4. Test group main clinical syndromes improve significantly better than the controlgroup (P <0.01). Test group single main symptoms of chest pain, heart palpitations, upset,breathlessness, not supine, leg edema significantly improved (P <0.05) than the controlgroup.5. Test of patients hypersensitive CRP, lipoprotein (a), D dimer,levels decreased (P<0.05) than before treatment. After treatment between the two groups, the test groupdecreased significantly (P <0.05). The angina pectoris improvement was significantlybetter than the control group (P <0.05). The low incidence of test group of major adversecardiac events (P <0.05).Conclusion:This paper presented with acute myocardial infarction after PCI reperfusion injury tothe heart meridian injury as a pathological basis, with blood stasis and toxin as the keypathogenesis.Using the method of yiqihuoxue、huatanxiedu、wenjingtongbi,Resultsprove, Huangqi Guizhi Wu Wu Decoction combined with conventional western medicinetreatment, can promote postoperative ST-segment, effective in improving the symptoms ofangina; reduce the incidence of arrhythmia, prevention and treatment of reperfusionarrhythmias; improve left ventricular ejection fraction and wall motion scorelowerNT-proBNP levels in vivo, prevention and treatment of cardiac remodeling and improvedleft ventricular systolic function; reduce the incidence of major adverse cardiac events.Chinese medicine can be effective in improving clinical syndromes can significantlyalleviate the main symptoms such as chest pain, heart palpitations, upset, lower limbedema. The results of this project show Astragalus Gui Zhi Wu Wu Decoction combinedwith Western conventional treatment, can effectively reduce the ET-1, vascularinflammatory factors CRP, Lp (a) levels, D dimer levels to prompt Huangqi Guizhi WuwuTonga flavor throughprotect the vascular endothelium and prevention of inflammation, toprevent the the microthrombi and improve blood clotting-slimming system functionsmechanism, effective treatment of acute myocardial infarction after PCI reperfusion injury.
