The prevalence of diabetes and the quantity of diabetic patients haveincreased sharply in recent years.Diabetes with complications broughtserious burden to the human health and social development.Through theepidemiological investigation in China,estimated prevalence of Chinesediabetes was9.7%over the age of20adults.The main factors to lethaldisability in patients with diabetes mellitus were diabetic macrovascularand microvascular complications.The pathological basis of cardiovasculardiseases is the hardening of the arteries,a variety of pathogenic factors,among which the damaged vessel wall and chronic inflammation andvascular sclerosis and stenosis is the main pathogenic factor.Therefore,has important clinical significance to find the risk factors associated withthese risk factors,the risk for macrovascular disease in patients withdiabetes and the occurrence and development of evaluation delay diabeticcomplications.Numerous studies have shown that vascular endothelial dysfunction isthe initial factor of vascular lesions.When endothelial cells were in theactivation,damage and (or) apoptosis,they release of endothelialmicroparticles (EMP).EMP has proinflammatory and procoagulant,changes in vascular function.In some pathological process,the number ofEMP will increase significantly,such as hypertension,coronary heartdisease,diabetes and other conditions.Carotid artery intima-media thickness(Carotid intima-media thickness,IMT) is a sensitive index of earlyatherosclerosis,so it can be used as prediction,evaluation of early indicatorsof diabetic angiopathy.Atherosclerosis is a chronic inflammation disease,including atherosclerosis.The theory is that oxidized LDL (OX-LDL)launched the inflammatory reaction of AS.Through the regulation of nuclear transcription factor (NF-κB), OX-LDL induced the inflammatoryresponse of endothelial cells.Therefore,NF-κB was the central link in ASinflammatory lesions,it also may be diabetic atherosclerosis (DiabetesAtherosclerosis,DAS) and one of the important mechanisms ofdevelopment.This experiment chose164cases of patients with type2diabeteshospitalized,according to the ultrasonic measurement of IMT were dividedinto two groups,hardening of the arteries group (120cases);nonarteriosclerosis group (44cases),through the IMT detection of the plasmalevel of EMP,NF-κB and ultrasonic detection of ELISA by flow cytometry,and collect relevant clinical index,risk factors by multivariate LRregression analysis of type2diabetic patients with Logistic arteryhardening.Compared with the non arteriosclerosis group,age,duration of diabetesarteriosclerosis group were longer (P<0.01);Arteriosclerosis group EMP,NF-κB,HbA1c,LDL-c,BMI levels are higher than non arteriosclerosisgroup increased (P<0.05).Univariate LR regression analysis indicated thatgender,age,Logistic,duration of diabetes,HbA1c,EMP,NF-κ B,LDL-c, BMI,the differences were statistically significant (P<0.05).Multivariate LRLogistic regression analysis showed EMP,NF-κB and BMI wasindependent factor of atherosclerosis (EMP OR=1.788P=0.010, NF-κBOR=1.064P=0.035, BMI OR=2.204P=0.023).In summary,EMP,NF-κ B and BMI,as independent risk factors foratherosclerosis,can be used to predict,evaluate the type2diabeticmacrovascular disease in patients with risk. That is to say, In the case ofother conditions unchanged: patients with type2diabetes, BMI increase1Kg/m2, carotid intima-media thickness increasing the relative risk of1.204;EMP level1%increase, carotid intima-media thickness increased relativerisk of0.788; NF-κB every increase1ng/L, carotid intima-media thicknessincreased relative risk of0.064.
