Objective:Laparoscopic treatment has become the first choice in non-giant benign ovarian tumor operation. However its disadvantages, like contents from ruptured tumor cause chemical peritonitis and peritoneal granuloma formation, especially contents from ruptured ovarian mucinous cystadenoma spread and plant in peritoneum causing peritoneal pseudomyxoma, limit its application. This study compared the effect and prognosis of laparoscopic and traditional open surgery to resect ovarian mucinous cystadenoma, and thus come to the advantages and disadvantages of laparoscopic surgery in the treatment of ovarian mucinous cystadenoma.Methods:122cases with ovarian mucinous cystadenoma, treated with surgery, admitted in Linyi People’s Hospital from July2007to July2013,were retrospectively investigated. The indicators of laparoscopic group and laparotomy group, like Blood loss, operation time, postoperative complications, recurrence rate and cancerization rate, were compared.Results:122patients were successfully operated without intraoperative injury. In laparoscopic group, operation time was51.4±9.1min, blood loss was30.1±10.2ml; while in laparotomy group, operation time was60.1±9.5min, blood loss was45.8±15.6ml. The difference was significant (P<0.05).35cases of laparoscopic surgery have no conversion to laparotomy, postoperative bleeding, organ damage, chemical peritonitis and subcutaneous emphysema and other complications. Among the87cases of laparotomy,2cases with postoperative fever, including one case of tumor excision in cesarean section.114patients were followed up and no cancerization was found.14patients were with tumor recurrence (4cases of laparoscopic group,10cases of laparotomy group). Cancerization rate was0, total recurrence rate was12.3%. The average age of recurrence was34(20-39) years, the average time to recurrence was31.2(6~55) months. In laparoscopic surgery group, the recurrence rate was11.4%, while in laparotomy group, the recurrence rate was12.6%, which had no significant difference (P>0.05).Conclusion:Recurrence rate of laparoscopic group and laparotomy group had no significant difference; operation time, blood loss of laparoscopic group were lower than that of laparotomy group, and had significant difference. Laparoscopic surgery for ovarian mucinous cystadenoma treatment is safe and effective, and its clinical application is worthy of promotion.
