Objective: To analyze the related factors affecting the recurrence and prognosis of benign borderline and malignant mucinous ovarian tumors,so as to provide help for clinical diagnosis and treatment.Method: The clinical data of 560 patients with mucinous ovarian tumor admitted to the fourth hospital of He Bei Medical University from January 2008 to December 2017 were retrospectively analyzed,Among them,there were 368 cases of mucinous cystadenoma,90 cases of bord-erline mucinous cystadenoma,and 104 cases of mucinous cystadenocarc inoma.The age of initial diagnosis,symptoms,serum tumor markers bef ore surgery,surgical methods,surgical approaches,intraoperative tumor r-upture,ascites,surgical pathological stages,and postoperative adjuvant c hemotherapy were analyzed,and so on,The factors affecting the recurre nce and prognosis of mucinous ovarian tumor were analyzed.Patients' p ostoperative recurrence,treatment and survival after recurrence were und erstood by telephone and outpatient review.SPSS software(version 21.0)was used to establish a database and conduct statistical analysis.Results:1.Patients with mucinous cystadenoma were followed up for an av erage of 56.8±28.95 months(12-123months),and a median of 57 month s,During the follow-up,26 patients relapsed,the percent of recurrence rate was 7.1,The mean recurrence time was 36.96±18.54 months(4-75months),and the median recurrence time was 36 months.and no death related to the disease was found.factors associated with platinum resista nt relaps by univariate analysis were as follows: The recurrence of muci nous cystadenoma is not related to the tumor rupture in the surgical ap proach(laparoscopic,laparotomy),the tumor diameter and size,and the level of preoperative tumor markers(P>0.05),The recurrence was ass ociated with the removal of myxoid cystadenoma(P<0.05).2.Patients with borderline mucinous cystadenoma were followed up for an average of 46.5±30.32 months(12-126months),and a median of 34.5 months.During the follow-up,11 patients relapsed,the percent of recurrence rate was 12.2,The mean recurrence time was 23.45±17.83 months(3-60months),and the median recurrence time was 21 months.and no death related to the disease was found.factors associated with platinum resistant relaps by univariate analysis were as follows: The recurrence was associated with fertility preservation surgery and infiltration(P < 0.05),It was independent of age,preoperative tumor markers,tumor pathological type,postoperative chemotherapy,intraoperative appendectomy and lymph node resection(P > 0.05).Multivariate analysis showed: Surgical method(P=0.030,OR=13.529,95%CI=1.294-141.504)and infiltration(P=0.001,OR= 18.797,95%CI=3.083-114.602)were independent risk factors for recurrence.3.Patients with mucinous epithelial ovarian cancer were followed up for an average of 58.54±34.18 months(12-128months).During the follow-up,17 patients relapsed,the percent of recurrence rate was 19.1,The mean recurrence time was 22.53±15.35 months(4-53months),and the median recurrence time was 17 months.factors associated with platinum resistant relaps by univariate analysis were as follows: The recurrence was related to FIGO stage,postoperative residual lesions,preoperative ascitic volum?1000ml,and the number of chemotherapy courses(P<0.05).It was independent of age,preoperative tumor markers,tumor diameter and intraoperative appendectomy had no effect on the prognosis(P > 0.05).Multivariate analysis showed: Postoperative residual lesions and chemotherapy duration were independent risk factors for recurrence(P<0.05).4.During follow-up,8 patients with mucinous epithelial ovarian cancer died,The mean survival time was 75.64±32.91 months(9-127 months),and the median survival time was 77 months,Univariate analysis of prognosis revealed: FIGO stage,postoperative residual lesions,ascites volume ?1000ml and sufficient chemotherapy course were risk factors affecting the 5-year survival rate of patients(P<0.05),However,age,tumor diameter,preoperative CA125 were normal,Intraoperative appendectomy and lymph node resection had no effect on the prognosis(P>0.05).Multivariate analysis showed that: FIGO stage and full course of chemotherapy were independent risk factors for 5-year survival(P<0.05).Conclusion:1.Removal of myxoid cystadenoma is a risk factor for recurrence of myxoid cystadenoma of ovary.2.Fertility preservation surgery and tumor infiltration are risk factors for ovarian borderline mucinous tumor recurrence.3.Late FIGO stage,residual lesion,preoperative ascites more than 1000 ml and insufficient chemotherapy course are risk factors for recurrence and prognosis of mucinous epithelial ovarian cancer.4.Ovarian borderline mucinous tumors are at risk of developing into mucinous epithelial ovarian cancer,and strict follow-up is required for such patients.
