Objective:To investigate the clinical characteristics of eosinophilic gastroenteritis, including diagnostic criteria, differential diagnosis, treatment, in order to improve the quality of diagnosis and therapeutic effects of eosinophilic gastroenteritis.Methods:The Clinical data of one patient diagnosed with eosinophilic gastroenteritis were analyzed retrospectively and the correlation literature were reviewed.Data:A middle-aged male patient, sustained abdominal distension more than20days. Prior to admission, in the other hospital WBC, eosinophils count and transaminase elevation have been tested, all of them were increased. Ascites has been determined using ultrasonography, and bloody ascites has been observed by abdominal puncture. Ascites smear showed a lot of eosinophils granulocyte and RBC, as well as mesothelial cells. After symptomatic treatment, symptoms were remained and the condition did not improve.Interventions:The conventional examination, chest radiological imaging, abdominal CT test, bone marrow biopsy, abdominal paracentesis, endoscopy, colonoscopy and pathological examination have been implemented to confirm the diagnosis. Regular treatment including diuretic, nutritional support have been given in the pocess of diagnosis. Once the diagnosis was clear, oral prednisone (35mg per day) was carried out, and stopped gradually after5weeks.Results:The pathological diagnosis results of patient with abdominal distension were shown as follows:(esophagus) squamous hyperplasia with dysplasia (mild);(gastric antrum) chronic inflammation of mucous membrane;(duodenum) mucosa in chronic inflammation, the tissue is infiltrated extensively with lymphocytes, plasma cells and a few neutrophils, eosinophils;(colon) chronic inflammation of the mucous membrane, infiltrating a small amount of eosinophilic granulocyte. Other organs except gastrointestinal tract were not infiltrated with eosinophilic granulocytethe, and other diseases which induce eosinophilic infiltration were excluded. Finally, the patient was diognosed with eosinophilic gastroenteritis. Conclusion:For the diagnosis of patients with ascites, we should find out causes from common illnesses frist, but can not ignore some rare diseases such as eosinophilic gastroenteritis. The patients with eosinophilic gastroenteritis are successfully treated with hormone. Physicians should raise awareness of the disease, reduce misdiagnosis and missed diagnosis.
