Objective To investigate the relationship of serum cystatin C with diabetic nephropathy among type2diabetic patients.Methods A total number of115type2diabetic patients who were hospitalized in the First Affiliated Hospital of Guangxi Medical University were analyzed retrospectively.Two groups were divided according to the rate of24hours urinary albumin excretion (24h-UAE).A group-patients whose24h-UAE<30mg belonged to normal albuminuria group,B group-The ones whose24h-UAE≥30mg belonged to type2diabetic nephropathy.The clinical characteristics were then compared. The retated fators of DN were analyzed. With or without diabetic nephropathy as the dependent variable, Age, BMI, SBP, DBP, DM duration, TC, TG, HLD-C, LDL-C, HbA1c, BUN, Scr, Sua, GFR, Cys-C as independent variables wereanalyzed by Logistic regression analysis, To find out the factors that may affect the type2diabetic nephropathy. Results1. Serum cystatin C concentration in elderly patients with type2diabetes than non-elderly patients was significantly higher; cystatin C in different populations gender are the same; in the group with BMI as a reference, the standard weight and less, overweight and mildly obese, moderately obese and above its serum cystatin C concentration was undifferentiated; Serum cystatin C concentration in the two groups whose in order to glycosylated hemoglobin were undifferentiated.2.CysC, UAE in patients with type2diabetes with nephropathy or not in comparison difference was statistically significant, from the stage to the normal urinary albumin persistent microalbuminuria, macroalbuminuria period, cystatin C,24h urinary albumin excretion rate was significantly increased.3. Type2diabetes’BMI and24h-UAE in a negative relationship on the level of P<0.05, the correlation coefficient is0.215, Cys-C is positively correlated with24h-UAE on the level of P<0.01, the correlation coefficient is0.288.4.Type2diabetes patients ages, Scr, Sua, Cys-C are related to GFR negative relationship, correlation coefficients are0.416,0.712,0.279,0.314.5. Type2diabetes age, SBP, Sua, TG, UAE are positively correlated with Cys-C, the correlation coefficients are0.454(P<0.01),0.304(P<0.01),0.238(P<0.05),0.204(P<0.05),0.288(P<0.01), and a negative correlation between GFR and Cys-C, the correlation coefficient is0.304(P<0.01).6.with or without renal disease as the dependent variable, age, SBP, DBP, DM duration, TC, HLD-C, HbAlc, Scr, Sua, GFR, CysC as independent variables to Logistic regression analysis the results showed that DM duration and cystatin C is a risk factor for type2diabetic nephropathy (OR values were1.007,5.605, P<0.05).Conclusion:1.Serum cystatin C may vary with increasing age.2.Duration, serum cystatin C may be a risk of factors for type2diabetes nephropathy.
