Objectives1.Research the serum cystatin C (Cys-C) levels of in the coronary atherosclerotic heart disease (CAD) patients,which was diagnosed by coronary angiography (CAG).2.Research the differences levels of serum Cys-C in CAD patients with different types and different clinical severity.3.Evaluation the relationship of the serum Cys-C levels and the severity of coronary heart disease.Subjects and methodsCollecting125cases of patients who were hospitalized in cardiovascular department of Northern Jiangsu People’s Hospital from June2013to February2014, they all have completed coronary angiography examination. According to the result of the Coronary arteriography(CAG) divided into coronary heart disease(CAD) group (n=100) and the non-CAD group (n=25). The CAD group divided into stable angina pectoris (SAP), unstable angina pectoris (UAP) and acute myocardial infarction (AMI) according to the clinical types. Comparing the differences of serum Cys-C level between the non-CAD group and CAD group. At the same time the differences of serum Cys-C level between different clinical subgroups. According to the CAG results, the CAD group was divided into one,double and three pathological changes,then comparing the differences of serum Cys-C level between non-CAD group and CAD group. The same comparison was done between different subgroups.Each patient of the group are regular recorded general information, such as sex, age, history of smoking, history of drinking,hypertension, diabetes,et al. Collection the peripheral venous blood, text biochemical indicators of fasting plasma glucose (FPG), triglycerides (TG), total cholesterol (TC), high-density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C), creatinine (Cr), urea nitrogen (BUN), serum cystatin C (Cys-C), myocardial enzymes or myocardial-injury markers,et al. Every patient do the conventional electrocardiogram (ECG) examination.Results1. The general information in control group and stable angina pectoris group, unstable angina pectoris group and acute myocardial infarction group was in the positive relationship with sex ratio, the proportion of high blood pressure and FPG (P<0.01), but in negative relationship with age, smoking history, TC, TG, HDL-C, LDL-C, BUN and Cr(P>0.05).2. The serum Cys-c levels were higher in stable angina pectoris group(SAP), unstable angina pectoris group(UAP) and acute myocardial infarction group(AMI) compared with those of control group,the differences all have statistical significance (P<0.05). And unstable angina pectoris group,acute myocardial infarction group compared with control group respectively, the differences all have remarkable statistical significance (P<0.01). The serum Cys-c levels in stable angina pectoris group, unstable angina pectoris group and acute myocardial infarction group increased sequentially, unstable angina pectoris group compared with stable angina pectoris group,acute myocardial infarction group compared with unstable angina pectoris group,the differences in the serum Cys-c levels all have not statistical significance(P>0.05). Acute myocardial infarction group compared with unstable angina pectoris group, the differences in the serum Cys-c levels have remarkable statistical significance(P<0.01).3. The general information in control group and single lesion vessel group, double lesion vessels group and three lesion vessels group was in the positive relationship with sex ratio, the proportion of high blood pressure (P<0.01), but no statistical significance in age, smoking history, BUN, Cr,FPG, TC, TG, HDL-C and LDL-C(P>0.05).4. The serum Cys-c levels were higher in single lesion vessel group, double lesion vessels group and three lesion vessels group compared with those of the control group, the differences all have remarkable statistical significance (P<0.01). The serum Cys-c levels in single lesion vessel group, double lesion vessels group and three lesion vessels group increased sequentially, three lesion vessels group compared with double lesion vessels group, double lesion vessels group compared with single lesion vessel group,the differences in the serum Cys-c levels all have not statistical significance(P>0.05). Three lesion vessels group compared with single lesion vessel group, the differences in the serum Cys-c levels have remarkable statistical significance (P<0.01).5. The Gensini score was significantly higher in stable angina pectoris group, unstable angina pectoris group and acute myocardial infarction group compared with that in control group, the differences all have remarkable statistical significance (P<0.01). The Gensini score in acute myocardial infarction group was significantly higher than that in stable angina pectoris group, unstable angina pectoris group, the differences all have remarkable statistical significance (P<0.01). Unstable angina pectoris group compared with stable angina pectoris group, the differences in Gensini score have not statistical significance(P>0.05).6. The Pearson correlation analysis was used between the serum Cys-C levels of different patients in clinical types of coronary heart disease and Gensini score, the resules showed that the serum Cys-C levels and Gensini score in patients who with coronary heart disease was significantly correlated(r=0.58,p<0.01),the serum Cys-C levels were positively correlated with Gensini score.ConclusionsThe serum Cys-C can be used as an reference indicator of coronary heart disease in clinical classification and severity of coronary artery disease. The serum Cys-C levels and coronary artery disease was positively correlated, and with the increase in coronary artery lesions, the serum Cys-C levels rise.
