Objective:To study the clinicopathologic factors influencing outcomeof huge primary liver cancer(HPLC) in patients undergoing hepatectomy.Methods:A retrospective clinical analysis was made in120eases ofHPLC,admiaed to our hospital from January2006to January2012,andfollow-up was conducted by Out-patient and telephone connections or thepetition,18post-resection prognostic factors were assessed by SPSS17.0software,and univariate analysis and multivariate analysis was done jnorderto determine clinicopathologic factors which had influenced theprognosis of HPLC patients after hepatectomy.Results:Up to now,In120cases,9cases were losed(the rate of follow-upwas92.5ï¼…),96patients(80%) were dead in a11.Our datashowed that the1,2,3,5years cumulative SR in total group of hepatectomy Was81.2%ã€54.2%ã€39.0%ã€16.1%and median survival time in patients with HPLC was28months.Univariate analysis showed that preoperative liver function,capsule, intrahepatic metastasis, vascular invasion,cirrhosis affected,radical resection postoperative survival significantly.The Coxmultivariate model indicated that independent prognostic factors werevascular invasion,intrahepatic metastasis,radical resection.Conclusions:Intrahepatic metastasis and vascular invasion are theclinicopathologic factors influencing outcome of huge primary livercancer(HPLC) in patients undergoing hepatectomy;Improving curativeresection rate of HPLC significantly extend the survival time ofpatients.Postoperative survival rates is low in patients with poor liver function. |
