ObjectiveTo develop team-based interventions provided by collaboration of hospital and community which focusing on functional exercises and psychological support for aged hip fracture patients. To evaluate the effect on limb function and activity of daily living.Methods92 aged hip fracture patients meeting to the criterion were recruited and divided into intervention and control group by cluster randomization. There were no statistic differences in gender, age, education level, fracture site, operation method and days in hospital. Patients of intervention group were given continous team-based nursing interventions including constructing groups, training, making a plan for Rehabilitaton handbook for hip fractured patients, rehabilitation guidings in community periodically,specific functional exercises guiding, home visit and telephone consulation for more than six months in addition routine care, while the control group only received routine care. Harris Hip Score and GDS were assessed upon discharge,1st month after operation,3th month after operation,6th month after operation.Results①The total score of Harris Hip Score were significantly higher in the intervention group than in the control group at 1st month after operation,3th month after operation and 6th month after operation (p<0.05). The score of pain domain, function domain and ROM domian of Harris Hip Score were significantly higher in the intervention group than in the control group at 1st month、3th month and 6th month after operation (p<0.01), The results of Chi-square test indicated that no statistic difference was evidenced on the joint deformity domian of Harris Hip Score between the intervention group and the control group.②The score of GDS were significantly higher in the intervention group than in the control group at 1st month after operation,3th month after operation (p<0.05). ③Qualitative Research was used to describe the assessment of the team-based interventions.ConclusionThe team-based interventions provided by collaboration of hospital and community focusing on functional exercises and psychological support could help to promote the hip function and depression level of aged hip fracture patients. Therefore this team-based continous nursing intervention is an effective approach in improving the physiology and psychology of aged hip fracture patients. The team-based interventions received high opinion.
