Objective:discuss the correlation between serum homocysteine level and arteriosclerosis for Uygur and Han patients with H-type hypertension.Methods:513 essential hypertension patients diagnosed and hospitalized from January 2011 to December 2013 were selected as the research objects, including 252 patients with H-type hypertension (Hcy≥0 μmol/L),261 patients with simple hypertension (Hey<10 μ mol/L),194 Uygur patients and 319 Han patients. The patients’gender, age, height, weight, smoking and drinking history, duration of hypertension, biochemical indicator and other clinical data were collected. Limb Doppler was applied to inspect the patients’ baPWV, the ambulatory blood pressure was tested, PPI and circadian rhythm of blood pressure were calculated and the relationship among homocysteine, baPWV, PPI and circadian rhythm of blood pressure in Xinjiang Uygur and Han nationalities was analyzed. Results:(1) Hcy level, age, baPWV and PPI in Uygur patients with H-type hypertension were higher than those in the simple hypertension group (P<0.05); while the decrease rates of nighttime systolic and diastolic pressure were lower than those in the simple hypertension group, and the difference was statistically significant (P<0.05); (2) Hcy level age, duration of hypertension, smoking rate, male constituent ratio, baPWV,24Hpp and PPI in Han paitents with H-type hypertension were higher than those in the simple hypertension group (P<0.05); while the decrease rates of nighttime systolic and diastolic pressure were lower than those in the simple hypertension group, and the difference was statistically significant (P<0.01);(3) In the comparison between Uygur patient group with simple hypertension and Han patient group with simple hypertension:BMI, glycosylated hemoglobin, male constituent ratio,24hSBP,24hPP and PPI in Uygur patient group were higher than those in Han patient group, the HDL was lower, and the difference was statistically significant (P<0.05); In the comparison between Uygur patient group with H-type hypertension and Han patient group with H-type hypertension:BMI, glycosylated hemoglobin,24hPP and PPI in Uygur patient group were higher than those in Han patient group, homocysteine level was lower, and the difference was statistically significant (P<0.05); (4) the logistic regression analysis result with the dependent variable of Uygur patients’ arteriosclerosis-related indicators can show:age (OR=3.40,95%CI 1.62~7.15; OR=4.26,95%CI 1.98~9.16) and duration of hypertension (OR=2.45,95%CI 1.26~4.75; OR=2.04,95%CI 1.11~3.78) were risk factors for the increase of baPWV (P<0.05); age (OR=3.47,95%CI 1.35-8.89) and HbAlc (OR=3.09,95%CI 1.02~9.41) were risk factors for PPI; The logistic repression analysis result with the dependent variable of Han patients’ arteriosclerosis-related indicators can show:Hcy (OR=1.94,95%CI 1.03~3.60; OR=2.51,95%CI 1.35~4.65) and age (OR=3.33,95%CI 1.94~5.69; OR=3.25,95%CI 1.91~5.49) were risk factors for the increase of baPWV (P<0.05); Hcy (OR=3.79,95%CI 1.36~10.59) and age (OR=2.38,95%CI 1.08-5.21) were risk factors for PPI; Hcy (OR=1.82,95%CI 1.05~3.13) and age (OR=1.78,95%CI 1.14~2.78) were risk factors for the decrease rate of nighttime systolic pressure; Hey (OR=2.20,95%CI 1.35~3.59) was the risk factor for the decrease rate of nighttime diastolic pressure.Conclusion:The atherosclerosis degree for Uygur and Han patients with H-type hypertension is more severe compared with the patients with simple pressure, and there is a difference between Uygur and Han nationalities; the increase of homocysteine level is a main risk factor for the atherosclerosis in Han patients with H-type hypertension, and the early intervention shall be conducted according to the different clinical characteristics in different nationalities to delay the progress.
