Abstract/Summary:
Objective To examine the risk of cement leakage in vertebroplasty.with the use of high-viscosity cementMethods This research contains two majar parts. First, we retrospectiveiy analysis the data of plain X ray and CT scan images of 68 patients who undergo vertebroplasty in the general hospital of PLA in the past 2 years, then evaluate the risk of cement leakage and the common segment where the cement leakage mostly occured; Second part is that 24 cadaver verterbral bodys which divided into 2 groups were selected to manufacture the model of vertebral compression fracture. High-viscosity cement and the low-viscosity cement was injected into the vertebral body respectively. Observe the risk of cement leakage by B.A.Georgy classification and the restore condition of the height of anterior walls between two groups. Then,16 landraces, average weight 45kg, was selected to manufacture the model of cement leakage by surgery and divide into 2 groups, after the high-viscosity cement and the low-viscosity cement was injected into the vertebral body respectively, evaluate the risk of cement leakage across the plain X-ray and CT scan images by B.A.Georgy classification. We sent the blood sample which got from the inferior vena cava to clinical labroatory to detective the concentration of calcium, phosphorus and magnesium before the surgery,1 and 3 month after the surgery, respectively. Those 16 landraces were killed after 3 months of sugery to get the specimens and observation the bone-cement juncture and the HE staining result of heart myocardium, liver and kindey.Results The result of retrospective analysis shows that the risk of cement leakage was 52.7% and the most common segment was T12 and L1; The cadaver spine experiments shows the results that the high-viscosity cement group had the lower risk of cement leakage (grade Ⅰ, Ⅱ, Ⅲ was 8,3,1, respectively) compared with the low-viscosity cement group(grade Ⅰ, Ⅱ,Ⅲ was 2,6,4, respectively), and the restore of the height of anterior walls was 94.3%(high-viscosity cement) and 93.5%(low-viscosity cement). And the result of animal experiment shows the same outcomes of cadaver experiment. The concentration of calcium, phosphorus and magnesium were no statistic difference among two groups before the surgery,1 and 3 month after the surgery, respectively. And the bone-cement juncture was nearly same.Conclusions Compared with low-viscosity cement in verterbraplasty, high-viscosity cement was much safer and comes with less complications. And two groups of cements has the same effect of anterior walls’ height restore and the same results of concentration of calcium, phosphorus and magnesium, then comes with the same result of bone-cement juncture. So we can clearly claim that high-viscosity cement was a good choice of VPs and may have a brightly future.
