Observation On The Effect Of Poking Reduction External Fixation For Comminuted Fractures Of The Distal Radius

Objective:Based on the manual reduction and poking external fixator for the treatment of comminuted fractures of the distal radius, for the clinical treatment of comminuted distal radius fractures provide the basis for clinical treatment planning.Method:In the First Affiliated Hospital of Heilongjiang University Of Chinese Medicine Department of Harbin fifth hospital were collected and two of bone, From 2014 March-2014 year in December by poking reduction and external fixation for the treatment of comminuted fractures of the distal radius,According to the order of admission will meet the inclusion criteria (50 patients were AO type C2, type C3), were randomly divided into treatment group and control group,Each group had 25 patients, male 18 cases, female 32 cases, age 41-82 years old.The cause of injury:41 cases of falls, fall from height in 5 cases,4 cases of other injuries.Two groups were treated with general treatment in hospital.Treatment group: manipulative reduction and external fixation for the treatment of prying.Control group:open reduction and internal fixation with plate.On two groups of follow-up period of joint activity, fracture reduction Radiology (palmar tilt, ulnar deviation, radial height), the modified Shea wrist score system and other aspects of statistical analysis, evaluation of two methods for the treatment of comminuted fractures of the distal radius effect on.Results:50 cases were followed up.According to the modified Shea score standard of fracture healing:The Control group of 25 cases, 12 cases were excellent, good in 7 cases,3 cases, poor in 3 cases, the excellent and good rate was 76%;There were 4 cases of superficial incision infection, the incidence rate was 16%,2 cases of skin flap necrosis after treatment, healing.Treatment group,14 cases were excellent, good in 8 cases,2 cases, poor in 1 cases, the excellent and good rate was 88%;There were 2 cases of pin tract infection,1 cases of skin necrosis healed after treatment, no severe malunion or nonunion, no osteomyelitis cases.Conclusion:Poking treatment with little trauma, less complications, stable fixation, early functional exercise, do not destroy the blood circulation, economic applicable around the fracture reduction and external fixation support, conducive to fracture healing, through the observation and comparison, poking reduction and external fixation is better than open reduction and internal fixation, in conformity with the biological fixation, and fracture healing fast, especially for patients with severe soft tissue injury, need long-term medication or combined with nerve, vascular injury patients.
