Objective:To explore the application values of DWI in the differential diagnosis of cerebral alveolar echinococcosis(CAE) and cerebral tuberculoma.Methods:Materials and methods:We retrospectively included 15 patients with CAE and 15patients with cerebral tuberculoma in the Xinjiang Medical University First Affiliated Hospital, from 2009 to 2015.All cases have integral clinical materials and were diagnosed on the basis of pathologic finding or clinical finding.The MRI equipment used was the American GE Excite Ⅱ 3.0 T superconducting MRI scanner, with an orthogonal head coil. Conventional MRI examination, enhancing imaging and diffusion-weighted imaging sequence (b-values =1000 s/mm2) were carried out on patients, respectively.Measured the apparent diffusion coefficient ADC) of the infection group focal area, edema area and the offside normal area(control group).The SPSS17.0 (SPSS Inc. Chicago, IL, USA) statistical analysis software package was used in the study. Analysis of ADC, rADC value changes in the infection group focal area, edema area of CAE and cerebral tuberculoma.Results:n this study, the average ADC and rADC value in the infection group focal area with CAE is 1.16 ±0.16,1.62 ±0.22; the average ADC and rADC value in the infection group focal area with cerebral tuberculoma is 1.25 ±0.27,1.59±0.51;the average ADC value compared between two kinds of infected focal substance area had statistical difference(t-2.95, P< 0.05); the average ADC and rADC value in the edema area with CAE is 1.17 ±0.15,1.63 ±0.24; the average ADC and rADC value in the edema area with cerebral tuberculoma is 1.19 ±0.28,1.54±0.50;the average ADC value compared between two kinds of edema area had statistical difference(t-3.01, P<0.05);the average ADC value of the edema area and infection group focal area with CAE compared had not statistical difference(P>0.05).The ADC value of the infection group focal area and edema area with cerebral tuberculoma higher than CAE. Conclusions:DWI combined with conventional MRI, providing valuable information for the diagnosis and differential diagnosis of CAE and cerebral tuberculoma.
