Part one Expression and significance of intefleukin-22, matrixmetalloproteinase-9 and migration inhibitory factor in the peripheralblood of inflammatory bowel disease Objective To test the expression of intefleukin-22(IL-22), matrix metalloproteinase-9(MMP-9) and migration inhibitory factor(MIF) in peripheral blood of IBD, so as to explore the correlation of IL-22, MMP-9, MIF with the activity of IBD, and the probable role of them in the pathogenesis of IBD.Methods A total of 80 patients with IBD were enrolled, among them, 43 Crohn’s disease(CD) patients were served as CD group, 37 ulcerative colitis(UC) patients were served as UC group. 40 healthy subjects were served as normal controls. The levels of IL-22,MMP-9, MIF in the sera were detected by enzyme-linked immunosorbent assay(ELISA).The multivariate logistic regression of IL-22, MMP-9, MIF for screening active stage CD group or UC group was performed to establish model of combined examination. The diagnostic performance of each individual or combined examination was evaluated by creating receiver operating characteristic curver(ROC).Results Compared with normal control group, the expression of IL-22, MMP-9 and MIF were significantly higher in peripheral blood of CD and UC groups(P<0.05), and there was no significant difference between CD and UC groups(P>0.05). The expression of IL-22, MMP-9 and MIF in peripheral blood of active stage CD and UC groups were significantly higher than remission stage groups(P<0.05), The expression of IL-22,MMP-9 were in multivariate logistic regression models of active stage CD and UC groups(P<0.05). Area under ROC curve(AUC) of individual examination for screening activeIBD was 0.747, 0.770, 0.699, in CD group, respectively, and was 0.837, 0.815, 0.761, in UC group, respectively, while AUC of combined examination was 0.853 in CD group,0.867 in UC group.Conclusions The expression of IL-22, MMP-9 and MIF is increased markedly in IBD patients, which correlates closely to the activity of IBD, and might play an important role in IBD generation and development.Part two The screening and characteristics of nutritional risk in 92patients with Crohn’s disease Objective To investigate the screening and to explore the characteristics of nutritional risk among patients with Crohn’s disease(CD) in Suzhou.Methods A total of 92 patients with CD were enrolled. The nutritional status of them were evaluated by Nutritional Risk Screening 2002(NRS 2002). The prevalence of nutritional risk was assessed. Then a multivariate logistic regression was performed to explore probable risk factors of patients with nutritional risk.Results The prevalence of nutritional risk was 77.2%(71/92). And the prevalence of nutritional risk was significantly differed in severity of disease, age at diagnosis(P<0.01), location of disease, behavior of disease and fistulizing disease(P<0.05), but was not in sex or history of bowel surgery(P>0.05). Severity of disease(OR=10.812,95%CI 2.737~42.718) and age at diagnosis(OR=0.943, 95%CI 0.897~0.992) were in multivariate logistic regression model of patients with CD who had nutritional risk.Conclusions The risk of malnutrition is high of patients with CD. Activity disease and earlier age at diagnosis are the risk factors of patients with nutritional risk.
