Clinical And Basic Researches Of Chronic Atrophic Gastritis

Objective:The incidence of chronic atrophic gastritis is increasing this year. Intestinal metaplasia(IM) and moderate or severe dysplasia often occurs with chronic atrophic gastritis(CAG), which often program as premalignant lesions. Currently the mode of stomach cancer is: chronic inflammatory response-atrophy-intestinal metaplasiadysplasia- stomach cancer. Stopping or even reduce CAG has become the key to block and reverse the development of precancerous lesions to gastric carcinoma. H.Pylori, as an important cause of CAG, get much attention in the treatment of chronic atrophic gastritis.Treatments based on syndrome differentiation of traditional Chinese medicine have many advantages in it. We focus this study on Hp-positive CAG, analyzing and summarizing researches concern, and explore the clinical effects of Traditional Chinese Medicine(TCM) Huatanxiaoyu decoction.Methods:1. Two reviews: the first one describes the modern medical researches of HP-positive CAG, focusing on research progress of pathogenesis and treatments of this disease; the second one is a summary of TCM researches on HP-positive CAG.2. To randomly group 130 case patients of HP-positive CAG into TCM therapeutic group, western medicine(WM) control group, 65 cases in each group. The therapeutic group takes TCM Huatanxiaoyu decoction two times per day and the medicines for Hp. WM group takes symptomatic treatment. Results:1. In Taditional Chinese medicine, CAG is equivalent to “distention and fullness”, “gastric abscess” and “Cao-za” etc. HP is like somewhat evil with a "poison" nature in TCM as an etiological factor of CAG. HP-positive CAG is deficiency in origin and excess in superficiality. The occurrence of various pathogenic factors leading to CAG.2. Total effective rates of symptom improvement, Endoscopy and pathologic change in therapeutic group is better than that in WM control group. The difference is significant(P <0.05); the curing rate of HP in therapeutic group is 75% and 77% in WM control group. The difference is not significant(P>0.05). Conclusion:TCM Huatanxiaoyu decoction could obviously improve the pathological conditions of gastric mucosa in patients with HP-positive CAG.
