Background:Pulmonary tuberculosis is a kind of the major infectious diseases, which is a critical threat to the public health. China is one of the 22 countries of heavy tuberculosis burden in the world. In terms of the number of tuberculosis patients, China ranked the second following the India. Because of the high infections, the long treatment cycle and heavy economic burden, tuberculosis patients were more likely to produce a series of psychological problems and aggravate inferiority feelings and other negative emotions. Self-identity is reduced. It is also not conducive to the healthy development of the self-esteem. Bad psychological state will have an influence on the disease occurrence, development and prognosis. The level of self-esteem will also affect people’s cognition, emotion, social behavior and motivation. The self-esteem and mental health are closely related. Therefore, We should understand the tuberculosis patients’self-esteem and psychology health present situation and its influence factors, and make correlation analysis of the patient’s mental health and self-esteem. Then we make targeted guidance. It is have important practical significance to the prevention and control of tuberculosis, improve patients life quality.Objectives:This study aims to explore mental health and self-esteem status and its influence factors among tuberculosis patients, as well as make correlation analysis of the patient’s mental health and self-esteem, in order to put forward some policies and suggestions to improve patient’s mental status and life quality.Data and Methods:We used multi-stage stratified cluster random sampling method in this study. At first, one city was selected respectively from each of western, central and eastern regions of Shandong Province randomly, including Jinan, Linyi and Weihai. Secondly, we ranked these counties in accordance with the level of GDP per capita, and classified as three categories, high, middle and low. Then, we used the simple random sampling method to select two countries from each sample city as the sample countries, including Zhangqiu, Changqing, Junan ,Yishui, Wendeng and Rushan. We investigated all tuberculosis patients enrolled from January 1st,2011 to December 31th,2011 in the 6 counties. We sent 800 questionnaires in this study,722 patients responded effectively, and the effective rate was 90.25%. Epidata3.1 software was used to input data and SPSS 17.0 was used to analyze the data.Main Results:1. K10 scores of 10-16 accounted for 41.27% points; 16-22 accounted for 25.07%; 22-30 accounted for 21.19% of the 30-50 accounted for 12.47%. The mental health status of the tuberculosis patients was not optimistic. The influencing factors were gender, feeling energy shortage recently or not, sleep quality, belonging to poverty or low income households, the severity of tuberculosis. The mental health status of female and poverty or low income households was worse than others. The mental health status of the ones with worse quality of sleep, more serious degree of tuberculosis and feeling energy shortage recently was relatively worse than others.2. SES score of 30 or more accounted for 57.62%; less than 30 accounted for 42.38%. The self-esteem of tuberculosis patients should be improved. The influencing factors were feeling energy shortage recently or not, the severity of tuberculosis, physical exercise status, culture level and economics status. Among them, the SES scores of the ones with feeling energy shortage recently, never physical exercise, more serious degree of tuberculosis, lower economics and cultural level were relatively lower than others, and their self-esteem situation was worse relatively.3. Spearman correlation analysis showed there was a significant positive correlation between psychological health and self-esteem status of tuberculosis patients (Spearman r=0.267, P=0.000). The more health psychology of tuberculosis patients is, the better self-esteem situation is relatively.Countermeasures and Suggestions:1.Plenty of propaganda education activities should be carried out actively, aimed at enhancing cognitive level.2.Medical institutions and personnel should play a very important role to improve the management level of patients.3.The country should increase the input of funds and policies to alleviate the economic pressure of the patients.4.The harmonious family and social atmosphere should be created to increase social support.
