BackgroundTuberculosis, as a type of chronic respiratory infectious disease, threatens human’s heath seriously. China was among the 22 countries with high TB burden, and had the second largest number of TB cases worldwide. TB restricted the development of economy and society with leading heavy burden to the government and peoples. Today, china had faced with a severe situation of TB. The number of the floating people who were fragile to the TB was so large that make the TB control strategies appropriate to the TB situation are summarized below. Based on the statistics report, there were 21.1 million floating people in the china in 2009, and it improved TB spread and challenged TB control program. In 2012, the number of floating people in shanghai was 9.6 million, made a 40% of population. Most of these persons came from central and western China, with 80% of TB patients of the country residing there. The floating TB patients were lack of TB knowledge, were in high burden of TB, and were more likely to be discriminated. Therefore, those patients may be in unhealthy mental condition, such as anxious, fear, loneliness and psychological obstruct. These factors will reduce the effect of the anti-TB treatments. To alleviate TB epidemic in floating people, a study on the mental health of TB patients are urgently needed. It will help provide better psychological support for the patients.Objectives1 Using a questionnaire that was adopted with SCL-90 in the floating TB patients who were diagnosed as TB, we aim to investigate the psychological status of pulmonary TB patients among the floating population.2 Impact factors that influence the score of SCL-90 were analysed in univatriate and multi-variate models respectively, and the factors related to the different was analysed.3 On the basis of the abosve study, psychological support strategies are proposed to tackle the mental problems.MethodsA survey using a structured questionnaire was conducted based on the scale (SCL-90) in the patients who were registered as TB between January 1,2013 and June 30,2013.SPSS 11.5 was used to conduct the data manage and analysis. Chi-square was used for catergorical data, while t test was used for continuous data. Generalized Estimating Equations (GEEs) were used to estimate the effects of potential risk factors of TB patients on mental problemResults1. A total of 338 migrant TB patients, who were diagnosed during the study period, enroled in the study. The patients were mainly male, aged between 18 and 29 and were from central and western china.2. The total score of SCL-90 of the migrant TB patients in Pudong District was lower than that of the normal.3. Among the migrant patients,19.82% had one or more kind of psychological problems; 12.72% of the patients were in obsessively compulsive status; 10.06% were in somatization status; 9.17% were in depression.4. The Generalized Estimating Equations (GEEs) indicated that the patients’ income level, age and history of TB were the influencing factors for the psychological status of the migrant TB patients.Conclusion1 The mental health status of migrant pulmonary tuberculosis patients in Pudong District was good certainly.2 The Generalized Estimating Equations analysis showed that level of the patients’ earning, history of TB and age were the influencing factors to the mental health status of the participants.3 We should provide psychological support for those patients who were retreatment, smear positive, brainworker.4 Strengthen economical support for the patients, and improve the effect of the anti-TB treatments.
