ObjectiveCerebral small vessel disease (CSVD) is onset occult, and its progress is slow. For a long time, its potential seriousness did not get enough attention, especially-the cognitive impairment caused by it, which we call it cerebral small vascular cognitive impairment (CSVCI).CSVCI is an important sub-type of vascular cognitive impairment (VCI). It is early stage of the disease, but once developed into the terminal stage of vascular dementia(VaD)is irreversible. So the treatment of CSVCI should focus on early intervention. Based on the previous research, HuayuTongluo Decoction can "removing blood stasis to produce new", HuaYuTongLuo Decoction can effectively improve Angiogenesis after cerebral infarction and the tissue perfusion of cerebral infraction, meanwhite can improve nervous function. Now we use The HuayuTongluo Decoction for CSVCI in order to expand its indication, through the observation of clinical efficacy of HuayuTongluo Decoction on the treatment of CSVCI, To improve the HuayuTongluo method may be the key to the treatment of CSVCI. The results will embody the advantages of treatment with Chinese medicine intervention; This is conducive to the development of effective delaying dementia compound drugs, and to provide experimental basis for clinical treatment of CSVCI.MethodAccording to inclusion, exclusion criteria randomized into sixty cases of CSVCI were divided into two groups, One is the treatment group and the other is the contrast group, each of them is thirty cases. Respectively, before treatment and after treatment for 30 days and 90 days using scale Mini Mental State Examination (MMSE)and the Montreal cognitive assessment scale (MoCA) to assess the overall cognitive function. Meanwhile Both group are taken the magnetic resonance imaging(MRI) before treatment and after treatment for 90 days, to evaluate the degree of white matter lesions, and to calculate the number of lacunar infarcts and cerebral micro bleeds. All the data are input to SPSS19.0 software to analyze.Results1.HuayuTongluo Decoction combined with modern western medicine to treat CSVCI, can effectively reduce the degree of cognitive impairment, we can see that is more effective than the use of Western medicine treatment;2. The treatment of HuayuTongluo Decoction combined with modern western medicine may slow down the trend of cerebral white matter lesion areas continue to expand and control the increase in number of lacunas ranges and cerebral microbleeds Compared with the contrast group;3. Under the collateral disease theory to treat CSVCI is an effective way of diagnosis and treatment;4. In the process of treatment, there are no adverse reactions associated with HuayuTongluo Decoction, so it is safe in clinical application.ConclusionsHuayuTongluo decoction combined with modern western medicine can effectively improve the cognitive functions in patients who is CSVCI, especially in executive capacity, the disorders of attention and directive force, and also we find that this treatment may slow down the trend of cerebral white matter lesion areas continue to expand and control the increase in number of lacunas ranges and cerebral microbleeds. The mechanisms may be are the HuaYuTongLuo Decoction can stimulate the vessels to revitalize blood to improve the blood perfusion, and then improve the chronic blood perfusion and protect the nerve fibers and myelin sheath, and reduce the destroy of cerebral small vessel wall. Compared with the contrast group, this treatment may slow down the trend of cerebral white matter lesion areas continue to expand and control the increase in number of lacunas ranges and cerebral microbleeds. and also help in the recovery of neurological functions, Finally reflected in the recovery of neurological functions.
