ObjectiveThis is a retrospective study.There is two treatment, one is the synov ectomy, the other is combined the synovectomy and the inactivated by plasm a knife and iodine. By comparing the clinical score and postoperative recu rrence of the case who take the different treatment, we try to find the mo-re effective way.MethodsThis study review 41cases of DPVNS of the knee that were radiologlically and pathologically diagnosed.There are two treatment for all the patients:18 patients were treated only by synovectom(the non-inactivated group),23 pati-ents were treated by synovectomy and the inactivated by plasma knife, iodi-ne (the inactivated group). All the patients, take the orthopedics knee exemin-ation before the operation, records the Lysholm score, the knee joint ROM (range of motion), MRI and X-ray image. After the operation. All the patients were followed up, and record at the time of last followed up, including the Lysholm score, range of motion, MRI image, recurrence rate. Through comparing the Lysholm score and knee joint ROM before the operation and the time of last followed up to.At the time of last follow-up, all patients take the knee MRI examination, in order to compare the recurrence rate.ResultAll patients were followed up for(30±1.2)month. There is no significant d ifference between two groups of patients by the analysis of the gender an d age. The non-inactivated group:preoperative Lysholm score (55.44±10.2 1),at the time of the latest follow-up(78+3.98),and there is a statistica lly significant difference(p<0.05). Comparing to the preoperative Lysholm, the last follow-up of the Lysholm score rise.The inactivated group:preop erative Lysholm score(42 ±6.50), at the time of the last follow-up(82±5. 61), and there is a statistically significant difference(p<0.05). Compare t o the preoperative Lysholm, the last follow-up of the Lysholm score rise.B ut there is no significant differences results of the preoperative Lyshol m and the last follow-up Lysholm score between two group, and the last fol low-up. The non-inactivated group:preoperative ROM(81.33+6.84)degrees, and the time of the latest fol low-up (130.11±9.20) degrees. there is a statisti cally significant difference(p<0.05).At the time of the last follow-up,t he knee joint activity is improved. The inactivated group:preoperative RO M(79.96±5.456)degrees, at the time of the last follow-up ROM(127.48±11.1 6) degree, there is a statistically significant difference(p<0.05).At the t ime of the last follow-up, the knee joint activity is improved. Compare tw o groups. But there is no significant differences results of the preopera tive ROM and the last follow-up ROM between two group.There are 6 patient s recurrence in the non-inactivated group t and only 2 in the inactivated group. There is a statistically significant difference(p<0.05). The inacti vated group receive the lower recurrence.ConclusionSynovial removal operation is an effective therapy in PVNS. Combining with the inactivated by iodine, ion scalpel can reduce the recurrence rate. It is a more ideal treatment method.
