Study On Measuring And Assessing Eye Lens Dose To Working Operator In The Interventional Radiology Suite

BackgroudRencently,In our country interventional radiology have become more and more popular,but given little attention to the problem on assessment and protection of the eye lens dose to working operator.In2011,ICRP made a major revision that the effects of deterministic threshold of the eye lens dose to working operator dropped to0.5Gy and equivalent dose limit should not exceed20mSv each year. So choosing the right way to measure the eye lens dose of the interventionalist will contribute to improving our ability to evaluate the biggest lens dose and making the corresponding protection.Objective(1)To compare the two dose equivalent value Hp (0.07) and Hp (3)(2)To estimate radiation dose to the eye lens of the interventionalist from five interventional procedures including CA (Coronary angiography)、PTCA (percutaneous transluminal coronary angioplasty)and PCI(percutaneous coronary intervention)、Cerebral aneurysm、Pacemaker Therapy and Radiofrequency ablation.Methods(1)The radiation doses to the eye lens for five interventional radiology fluoroscopic systems in four hospital were measured by using OSLs (Optically Stimulated Luminescence) dosimetry calibrated in Hp(0.07).For the eyes,the OSLs were positioned to the right,left and middle sides. Meanwhile,fluoroscopy times,CDs(cumulative doses),and DAPs(Dose area product) et.al were recorded for statistical analysis.(2)CA and PTCA+PCI procedures were doing in two hospitals using different machine,the number of the cases were about30in CA and15in PTCA+PCI,then we mrasured doses in the half of the number of the cases. The other three procedurs were choosed about15cases,measured doses about half of the cases as well.(3)Using Inlight200automatic machine to get the Hp(0.07) as the eye lens dose of the operators.According to the actual workload racords to estimate the eye lens dose in the different procedure.(4)Comparing the difference of the eye lens dose to operator in CAand PTCA+PCI in two hospital.(5)Comparing the two value of Hp (0.07) and Hp (3)(6)Using SPSS15.0to do the statistical analysis.Results(1)The maximum eye lens dose of operators in these five therapies are0.042、0.138、0.036、0.376and0.136mSv singlely,when the workloads are more than476,476,555,53,147cases respectively,an annual equivalence dose of the operators will exceed20mSv as recommended by the ICRP provided by the current study.(2)The left eye lens dose of the CA and PTCA+PCI in two hospital has no statistical deviation.(3)The value of Hp (0.07) and Hp (3) hase no statistical deviation(4)Single factor ANOVA analysis:Fluoroscopy times,accumulated doses,and DAPs(Dose area product) in different procedure have statistical deviation, P<0.001.The same eye lens dose of the different interventinalist in same therapy has no statistical deviation.(5)Making correlation comparing of fluoroscopy times,accumulated doses,DAPs and the eye lens dose:fluoroscopy times are related to accumulated doses,DAPs and the eye lens dose, accumulated doses are related to fluoroscopy times and DAPs,but show no correlation with eye lens dose; DAPs is related to fluoroscopy times,accumulated doses and right eye lens dose.(6)Multi-factor ANOVA analysis: Analysising Different interventioanlists, accumulated doses,the height of operators and eye lens dose;Under the invariable condition in other influencing factor,the eye lens doses in different operators are different,P<0.05,the doses between the two eyes or of the left eyes under different accumulated doses have statistical deviation, P<0.05.(7)Matched-pair analysis:The five procedures are divided in two groups.one includs CA、PTCA+PCI、Cerebral aneurysm、and Radiofrequency ablation,while the other only includs Pacemaker Therapy. The doses between the lefe eyes and the right eyes have no statistical deviation in Pacemaker Therapy, P>0.05,but the others are opposite,yet they have deviation, P<0.05.Conclusions(1)The eye lens dose of operators in Pacemaker Therapy are36.096mSv,which will receive an annual equivalence dose of more than20mSv,when they work more than53cases.(2)There are no statistical deviation in CA and PTCA+PCI theraphy in two hospitals.(3)Hp (0.07) andHp (3) have no statistical deviation,so we can make Hp (0.07) as the assessment values of the eye lens dose.(4)Fluoroscopy times.accumulated doses,and DAPs(Dose area product)in different procedures are different. The eye lens dose are different in differen procedurs,but the same eye lens dose of the different interventinalist in same therapy are almost close to each other. Under the invariable condition in other influencing factor,the eye lens doses between the two eyes or of the left eyes are different with the accumulated doses.(5)Fluoroscopy times is related to accumulated doses,DAPs and the eye lens dose, accumulated doses is related to fluoroscopy times and DAPs,but showed no correlation with eye lens dose.(6) The eye lens doses of operators are different under non-Pacemaker Therapy,but close to each other in Pacemaker Therapy.
