Objective:1. To study the distribution of traditional Chinese medicine(TCM) syndromes in patients with type 2 diabetes mellitus(DM) and hyperlipidemia.2. To analyze the influence of gender, age, duration of disease, body mass index(BMI), smoking and drinking on TCM syndromes.3. To investigate the association of fasting blood-glucose(FPG), 2 h postprandial plasma glucose(2h PG), glycosylated hemoglobin HBA1c(Hb A1c), total cholesterol(TC), triglyceride(TG), high density lipoprotein cholesterol(HDL-C), low density lipoprotein cholesterol(LDL-C), Apolipoprotein A I(Apo A1), Apolipoprotein B(Apo B) and the physical and chemical inspection index of the fatty liver disease(FLD) and the main syndromes in patients with type 2 DM and hyperlipidemia.Methods:The research object of this study consists of the patients with type 2 DM and hyperlipidemia that was outpatient or hospitalized in the Department of Endocrinology at Yunnan Provincial Hospital of Traditional Chinese Medicine from December 2013 to December 2014. After the collection of data on patients’ general information and the information from the four diagnostic methods, the syndromes were identified through TCM syndrome differentiation. Then the general conditions and the distribution of TCM syndromes of patients with type 2 DM and hyperlipidemia were obtained. In addition, the influence of gender, age, duration of disease, BMI, smoking and drinking on TCM syndromes was analyzed. The conclusions were obtained through statistical analysis.Results:1. Out of the 110 patients with type 2 DM and hyperlipidemia, 48 male(43.64%); 62 female(65.36%); the ratio between male and female: 1:1.3; age range: 36-75 years; mean age: 62.14±7.62 years, patients whose age range from 35 to 54 years are relatively less, while patients whose age range from 65 to 75 years are the most. The older the patients, the more cases with type 2 DM and hyperlipidemia. There are 36 patients who smoke and 19 patients who drink alcohol.2. Out of the 110 patients with type 2 DM and hyperlipidemia, the duration of type 2 DM ranges from 0.5 to 25 years with the mean duration of 9.2±5.10 years, while the duration of hyperlipidemia ranges from 0.5 to 20 years with the mean duration of 6.1±3.39 years. The BMI of these patients varies from 14.40 to 35.82kg/㎡ with the mean value of 24.89±2.75 kg/㎡.3. Out of the 110 patients with type 2 DM and hyperlipidemia, 15 patients(13.64%) have the syndrome of dampness heat of the spleen and stomach; 33 patients(30.00%) have liver-kidney yin deficiency syndrome; 56 patients(50.91%) have the syndrome of dual deficiency of qi and yin; 6 patients(5.45%) have the syndrome of yin deficiency and excessive heat; and no patient has the syndrome of dual deficiency of yin and yang; In the aspect of accompanying symptoms, 46 patients(41.82%) present with blood stasis syndrome and 8 patients(7.27%) present with phlegm and dampness syndrome.4. Out of the 110 patients with type 2 DM and hyperlipidemia, 73 patients have the complication of hypertension; 15 patients have the complication of coronary heart disease; 4 patients have the complication of cerebrovascular accident; 36 patients have no the abovementioned complications; and 77 patients have fatty liver disease.5. Out of the 110 patients with type 2 DM and hyperlipidemia, 60.91% of the patients present an increase in FBG with the mean level of FBG of 8.71 ±2.43mmol/L; 55.45% of the patients present an increase in 2h PG with the mean level of 12.02±3.64mmol/L; 53.64% of patients with an HBA1 c above 7.0% with the mean level of 7.74 ±1.49%; 66.36% of the patients show an increase in the TC with the mean level of 5.59 ±0.86mmol/L; 64.55% of the patients demonstrate an increase in TG with the mean level of 2.60 ±1.30mmol/L; 38.18% of the patients present an decrease in HDL-C with the mean level of 1.22 ±0.28mmol/L; and50.00% of the patients present an increase in LDL-C with the mean level of 3.36 ±0.70mmol/L.6. Age, duration of disease, BMI, smoking and drinking have no significant influence on the distribution of TCM syndromes(P>0.05), while gender influences the distribution of TCM syndromes(P<0.05).7. There is no significant correlation between the biochemical index of FPG, 2HPG, TC, TG, HDL-C, LDL-C, Apo A1, Apo B and TCM syndromes(P>0.05), while Hb A1 c has a correlation with TCM syndromes(P<0.05). The fatty liver disease has no significant correlation with TCM syndromes(P>0.05).Conclusions:The distribution of TCM syndromes in the 110 patients with type 2 DM and hyperlipidemia shows a gradual decrease from the syndrome of dual deficiency of qi and yin, the liver-kidney yin deficiency syndrome, the syndrome of dampness heat of the spleen and stomach to the syndrome of yin deficiency and excessive heat. No patients have the syndrome of dual deficiency of yin and yang. In the aspect of accompanying syndromes, patients accompanied with blood stasis syndrome are more than patients with phlegm and dampness syndrome. Age influences the distribution of TCM syndromes. To be specific, with the increasing in age, the proration of patients with from the syndrome of dual deficiency of qi and yin, the liver-kidney yin deficiency syndrome and the accompanying syndrome of blood stasis is increasing significantly. Among the biochemical indexes, only Hb A1 c has a correlation with TCM syndromes, which is seen in the result that the level of Hb A1 c in patients with the syndrome of dampness heat of the spleen and stomach is higher than that in patients with other syndromes.
