Research On Curriculum Development Of Community Nurses’ Continuing Education Based On Position Requirements

ObjectivesThe aims are to investigate position competence and needs of continuing education for community nurses in Zhangye; to formulate the analysis sheet of community nurse in Zhangye based on the needs of position; and to develop the curriculum of community nurses’ continuing education which is suitable to feature of health system in Zhangye, Gansu province.MethodsThis research was performed in three consecutive phases.The first phase was qualitative research on needs of continuing education for community nurses:12 managers in community health service centers and community health service stations were selected by using purposive sampling according to the phenomenological method, the data were collected by semi-structured interview and analyzed with seven step analysis of Colaizzi.The second phase was quantitative study on needs of continuing education for community nurses:(1)A cluster sampling of 167 community nurses(effective 163 cases) from 22 community health service institutions in Zhangye were selected in the study, who were investigated with self-owned questionnaire of the needs in community nurses for their continuing education. SPSS(version 17.0) was used for statistic description.(2)Convenience sampling of 439 community residents(effective 408 cases)from 3 communities in Ganzhou district and 1 community in other district were selected according to the population proportion in the urban in the study, who were investigated with questionnaire of the needs of community nursing service in community residents. SPSS(version 17.0) was used for statistic description.The third phase was curriculum development of continuing education based on the needs of position for community nurses:The draft of curriculum of community nurses’ continuing education was developed according to the interview and investigation on the needs of positions, and the other training programme. Then the expert group meeting, which was consisted of 8 experts from nursing colleges, health systems and community health service institutions, was used to revised the draft which designed basing on the quantitative and qualitative study and literature review.Results1 The results of qualitative research:There were six themes according to the managers in community health service centers and community health service stations. There were “deficiency of community nurses job necessary skills”, “the lacking job skills of community nurses”, “be more weighted towards theory than training”, “establishing the all-live study idea”, “The requirements were diversified in categories”, and “theory with practice and considering regional features and deficiency of features of local situation ”.2 The results of quantitative study:(1) The results of community nurses: 65.58% of community nurses would like to choose the mixed way of network and the centralized training; which the need most of the first three were Community rehabilitation nursing techniques, community of traditional Chinese medicine nursing technique, nursing new technology and new business knowledge; which the ability they should have the first three were basic nursing ability of Chinese medicine, interpersonal communication ability, rehabilitation nursing technical operation ability.(2) The results of community residents: 51.96% community residents knew little about the community service projects, among which 54.51% did not satisfied with the community service; which the need of disease nursing the most of the first three were emergency rescue and processing of accidental injuries, common diseases of therapeutic care, Nursing care and guidance of chronic diseases; which the need of disease prevention the most of the first three were regular check-up services, epidemic disease prevention and control in community, psychological care and counseling, which the need of health promotion the most of the first three were rehabilitation guidance and training of disabled people, sleep and health guidance, health movement guidance.3 The results of curriculum development of continuing education for community nurses :The draft of curriculum of community nurses’ continuing education was developed according to the results of part one and part two. And the curriculum of community nurses’ continuing education in Zhangye, Gansu province was revised by the expert group meeting. It included professional basic quality and ability, basic technology and ability, core technology and the ability, job quality and ability learning module; and it will last for 20 days with 160 periods, and be put into practice in stage of spring and autumn. If the community nurses can pass both the theories and practice assessment and they will achieve 25 credits on continuing education.ConclusionsThe continuing education of community nurses needs to be carried out step by step in a planned way. It should be more practical in contents, more diversites in ways and more flexible in time. Community residents knew little about the community service projects and did not satisfied with the community service, but the needs of community service was diversity,which was closely related to their lifestyle, family culture and the cognition of health. The curriculum of community nurses’ continuing education developed in the study was according to the diversity needs and expert group meeting, in order to promote the next continuing education of community nurses to implement in plan and smoothly.
