Background:Short for Intrauterine Device, IUD is one of the primary contraception for women in the child-bearing age, yet failure rate of IUD takers is higher in rural areas in China. Measures of improving technique and service level of family planning service institution effectively reduce the failure rate of using IUD. Studies analyze various factors that effect service quality of IUD placement and put forward Total Quality Management of IUD placement (IUD-TQM) from the perspective of management. Researches at home and abroad also show that the implement of IUD-TQM is able to enhance reproductive health and quality of family planning services. Through various measures of carrying out quality management group activities, employee involvement in TQM is a quite effective way of promoting the quality of family planning agency services at county level. The key to successful implementation of IUD-TQM are exploring the influence factors of employee involvement in TQM in family planning agencies at county level, motivating employee involvement in TQM and keeping sustained and effective employee involvement in TQM. The personal factors affecting employee participation in TQM is difficult to control, therefore, it is necessary to study the relationship between employee involvement and TQM from the aspect of organizational culture which affects employee values. There are overseas researches that have proved the correlation between organizational culture and employee involvement in TQM, yet there are few studies on organizational culture of family planning agencies at county level in China. Hence, in order to improve quality of family planning agency services at county level, it is very necessary to study the relationship between organizational culture and employee involvement in TQM.Objective:This paper is devoted to study the current situation of organizational culture, employee involvement in total quality management(hereafter refer to TQM),the influence of various demographic characteristics to organizational culture cognition and employee involvement, and correlation between organizational culture cognition and employee involvement in TQM of family planning clinics at county and town levels in China. In order to improve the service quality of family planning clinics at county and town levels, this paper will put forth relevant strategies and suggestions in favor of strengthening organizational culture construction and improving employee involvement in TQM of family planning clinics at county and town levels in China.Methods:Assessment scale of organizational culture and questionnaire of employee involvement in TQM is used to analyze data of 248 people with IUD from 4 counties and 12 towns of both Yunnan and Anhui provinces. Epidata 3.0 is adopted to establish the database and SPSS 17.0 is adopted to analyze data from scales and questionnaires mentioned above. The Investigation results is in the form of weighted mean difference. T-test is used to analyze between two sets of data, while variance analysis is used to analyze among multiple sets of date. Pearson analysis method is used on Correlation analysis. The result(P<0.05) is statistically significant.Results:1. Overall organizational culture of family planning clinics at county and town levels scores 3.9±0.8. Among the six dimensions of organizational culture, service object scores 4.2±0.8; activity scores 4.0±0.7; coordination and cooperation scores 4.0±0.9; organizational goals and planning scores 3.9±0.8; team orientation scores 3.7±1.0; equity scores 3.6±0.9. Various demographic characteristics of employees which include length of work, work intensity,education background, professional title, type of practice and training numbers are statistically significant to the difference of organizational culture cognition.2. Employee involvement in TQM of family planning clinics at county and town levels scores 3.8± 0.4. Among the nine dimensions of employee involvement, participation intention scores 4.2±0.4; perception of the utility scores 4.1± 0.5; self efficacy scores 3.5± 0.6. Employees of various demographic characteristics have no statistically difference in scores of employee involvement in TQM.3. Organizational culture of family planning clinics at county and town levels in dimensions of equity, activity, and team orientation and employee involvement in dimension of perception of authorization present positive correlation. Overall organizational culture and employee involvement in dimension of perception of authorization present positive correlation, but the correlation coefficient is small. Other dimensions of organization culture and employee involvement are not relevant.Conclusion:1. Degree of organization culture awareness of employees of family planning clinics at county and town levels belongs to superior middling. More attention should be paid to the dimensions of team orientation and equity, the key point of organization culture construction of family planning clinics at county and town levels. Organizational culture of family planning clinics at county level is superior to that at the town level. Demographic characteristics have an influence on organization culture cognition of employees of family planning clinics at county and town levels.2. During the progress of implementing TQM of family planning clinics at county and town levels, public participating is not so active.Various demographic characteristics of employees have no significant impact on degree of employee involvement. Publicity, education and training should be further strengthened to improve management system and to enhance activity of employee involvement.3. There is no significant correlation between the degree of employee involvement in TQM of family planning clinics at county and town levels and organizational culture cognition. However, there is a positive correlation among the three dimensions which include perception of authorization of employee involvement, overall organizational culture, and equity, activity and team orientation. In order to enhance the degree of employment involvement, measures in favor of increasing perception of authorization should be taken in the field of improving internal equity of family planning clinics at county and town levels, of advancing activity of employees and of boosting sense of community.
