The Study Of Clinical Efficacy Of Acute-on-chronic Liver Failure With Hot And Damp Stasis Syndrome By Integrated Traditional Chinese And Western Medicine Therapy

ObjectiveBy appyling the method of cooling blood and detoxification, qushi blood combined Chinese medicine retain enema treatment,this study treats ACLF Patients whose syndrome is a hot and damp stasis yellow, then evalute the clinical comprehensive efficacy in ACLF treated with combining traditional Chinese and western medicine therapy, and test patients endotoxin level, providing a powerful evidence for clinical risk factors for early detection of disease progression, blocking the disease progresses, preventing complications, reducing the case fatality rate and improving survival rateMethodsBy applying a open random controlled trial design,This study participants included 59 patients diagnosed as ACLF with hot and damp stasis syndrome. (cases from shenzhen hospital of TCM f rom 2012 to 2014 which participated in the national the "Eleventh Five-year" Major science and technology special issue" Study on acute and chonic liver failure of combining traditional Chinese and western medicine treatment"), patients were randomly divided into two groups, one group (treatment group) by integrated 39 people, western medicine treatment group (the control group) 20 people. Combine traditional Chinese and western medicine treatment group:According to the guidelines oral Chinese medicine decoction and traditional Chinese medicine retain enema on the basis of conventional treatment. Western medicine treatment group:Conventional treatment according to the guidelines. Contrastly analysis patients at 0 weeks,4 weeks,8 weeks with liver function, blood coagulation function, serum level of endotoxin (LPS), etc, to evaluate the clinical curative effect in ACLF patients treated with combining traditional Chinese and western medicine therapyResultsThrough observation of 8 weeks treatment, traditional Chinese and wester n medicine treatment group compared with the western medicine control group total effective rate of has significant difference(P<0.05).Two groups before treatment patients’ age, gender, biochemical indexes, blood coagulation fun ction, endotoxin levels are comparable at the same baseline. after 4 weeks,8 weeks,compare with conrol group,the treatment group of TBIL, DBIL signific antly reduced, especially at 4 weeks was better than control group, the diff erence was statistically significant (P<0.05). Experimental group PTA signif icantly increased than the control group, the INR significantly reduced, in 8 weeks was better than control group, the difference was statistically sign ificant (P<0.05). Experimental group ALT, AST ALB in 8 weeks was statistic ally significant difference than the control group(P<0.05). Integrative ther apy reduced levels of endotoxin in patients with better than western medicin e treatment group, the difference statistically significant (P<0.05).ConclusionsFor ACLF Patients with hot and damp stasis syndrome, combining traditional Chinese and western medicine treatment has better curative effect comparing with western medicine treatment group, especially in the case of fading jaundice and improve the function of blood coagulation, curative effect is particularly significant, as well as the improvement of the ALT, AST, ALB has certain advantages, at the same time can significantly reduce the patient’s level of serum endotoxin, therefor it has great significance for early prevention of endotoxin blood disease.
