Objective:To observe the curative effect of combinat of Chinese and Western medicine applite in the treatment of acute on-chronic liver failure (Hepatobiliary damp-heat).Methods:Select the48cases of patients with HBV-acute on-chronic liver failure (Hepatobiliary damp-heat) in Hubei Province Chinese Medicine Hospital Liver Center from Mar,2010to January,2012. Randomly divided the48patients into the treatment group and control group, each group of each24cases. The control group was treated with Western medicine treatment, and treat the treatment group with "addition and subtraction of Gan lu xiao du pill " besides the western applied in contrast group. Observing the Liver function、PTA、HBV-DNA、Clinical effects. Symptomatic effects. Incidence of complications and Mortality rate after4weeks of treatment.Results:1. The changes of Liver function、PTA and PT between the two groups before and after treatment:After4weeks the ALT, TBIL, PT, TBA were obviously decreased in the two groups, and ALT, TBIL, PT before and after treatment difference was significantiy statistical significance(p<0.01), TBA comparing data was not statistically significance(p>0.05). Including the treatment group TBIL, PT decline is more apparent, compared with the difference was statistically significant (p<0.05). two groups of patients, PTA, ALB,CHE,CHOL of are increased,The different was statistically significant, PTA,ALB,CHE, p<0.01, CHOL p<0.05. PTA,CHE treatment group increased even more, the difference was statistically significant (p<0.05).2. Four weeks after treatment for patients with two groups of integrated treatment effect and situation of death:treatment group clinical cure2cases, clinical better (16),and the total effective rate was75.0%, and the control group clinical cure in1, and clinical10cases, the total effective rate45.8%, two group total effectiveness is statistically significant diff ierence (p<0.05); The treatment group3cases of death, and mortality was12.5%, and the control group4cases of death, and mortality was16.7%, two groups of patients is not statistically significant differences between the mortality rate (p>0.05).3. The syndrome integral change after4weeks treatment:Before treatment,there were no statistical significance between two groups (p>0.05),but after4weeks treatment, the syndrome integral of tow groups were obviously decreased, the difference being statistically significant (p<0.01) compared with before treatment,also between the two groups after treatment(p<0.01).4. Four weeks after treatment for symptomatic curative effect is: treatment group clinical cure2cases, powerfully in14cases, effective in3, the total effective rate of79.2%; The control group clinical cure in1, significant effective in7cases, effective in5patients, and the total effective rate was45.8%, the treatment group average markedly improved significantly more than the control group, the symptoms and the total effective rate higher than those in the control group, the comparison between the two groups was statistically significant difference (p<0.05)5. Two groups of patients after four weeks HBV-DNA turn into negative:the treatment group of HBV-DNA Yin turn rate was45.8%, to41.7%in the placebo group, comparative differences between the two groups was statistically significant (p>0.05)Conclusion:1.Combinat of Chinese and Western medicine applite in the treatment of acute on-chronic liver failure (Hepatobiliary damp-heat) beneficial to patients with jaundice to retreat and blood coagulation enzymes improve the mobility, can improve patients with symptoms, reduce the total score symptoms, improving quality of life, and get better clinical curative effect and comprehensive sickness curative effect, worth in clinical promotion2.
