Clinical Observation Of Wenpi Decoction With Added Ingredients On Patients Of Pyemia With Acute Gastrointestinal Dysfunction

Object:This research is focus on the clinical efficacy and the internal affects of Wenpi Decoction with added ingredient on patients of pyemia with acute gastrointestinal dysfunction (The spleen Yang deficiency). In this study, the internal affects count on the variety of abdominal circumference, intravesical pressure, pH of gastric mucosa, score of intestinal failure and the motilin of serum. Through this research, we provide objective evidence of the practicability of Wenpi Decoction with added ingredient in the treatment of acute gastrointestinal dysfunction, then provide a better choice for patients with acute gastrointestinal dysfunction.Methods:This is a perspective study. In this study, all the patients of pyemia with acute gastrointestinal dysfunction were included, who were hospitalized in Traditional Medicine Hospital of Qingyuan during October 2013 to December 2014. The diagnostic criteria of pyemia is based on the 2nd edition of Emergency internal medicine pressed by People’s Medicine Publishing House, page 413-463. The diagnostic criteria of acute gastrointestinal dysfunction is based on Journal of Clinical Surgery(2013,21 (3), page159-161). The 60patients included in this research were randomly divided into experimental group and control group. There were 30 patients in each group. We provided each patient anti-infection, organ support and nutrition support, then we provide famotidine(20mg VD bid), domperidone(10mg nasal feeding tid) and nasogastic feeding as base treatment. In the experimental group, we provided Wenpi Decoction with added ingredient in addition. Wenpi Decoction with added ingredient included Rhubarb(15g), Angelica(9g), Rhizoma zingiberis(9g), Monkshood(6g), dang shen(15g), Glauber salt(6g), cauterized licorice(6g), fructus amomi(10g), Fructus aurantii immaturus(8g), Radix Platycodonis(6g), Astragalus membranaceus (20g), made into decoction (100ml) by dispensary of TCM in Traditional Medicine Hospital of Qingyuan. We feed the decoction through stomach tube once a day for 6 days. We recorded each patient’s history, asymptomatic, Laboratory tests, X-ray, ultra sound to make sure they were consistent to the diagnostic criteria of pyemia and acute gastrointestinal dysfunction. Then take the record of the variety of the score of gastrointestinal dysfunction, abdominalcircumference, intravesical pressure, pH of gastric mucosa and the motilin of serum. Base on all the information, we evaluated the global efficacy.Result:through the treatment, we find out something outstanding. The decrease of the score of gastrointestinal dysfunctionin each group is significant(P<0.01), The treatment group abdominal circumference no significant decline, Intravesical pressure have a downward trend, and the treatment group decreased more obvious, but without statistical significance. The increase of the motilin of serum and pH of gastric mucosa in each group is significant(P<0.01), and it’s more significant in experimental group than in control group(P<0.01). The treatment group was lower than those of control group in MODS incidence, mortality and ICU length of hospital stay, the difference was statistically significant (K0.05).Conelusion:Wenpi Decoction with added ingredients is an addition to the chemical medicine treatment for patients of pyemia with acute gastrointestinal dysfunction. It can significantly decrease abdominal circumference(P<0.01), but no significant difference function between two groups(P>0.05). And with Wenpi Decoction with added ingredients in treating patients with bladder pressure decreased compared with the control group. However, it were no significant in intravesical pressure and the score of intestinal failure between the two groups. By this treatment, symptoms of patients of Sepsis with acute gastrointestinal dysfunction can be improved(P<0.01). It can significantly increase their Ph of gastric mucosa and the motilin of serum compared to control group (P<0.01). In a word, Wenpi Decoction with added ingredients can be used as a part of treatment to patients of pyemia with acute gastrointestinal dysfunction. And can reduce the incidence of MODS and mortality and shortening the time of ICU in hospital.
