The Influence Of Tiaowei Chengqi Decoction In Critical Pa-Tients With Gastrointestinal Dysfunction

Background:Gastrointestinal dysfunction is a common complication in critical pa-tients in ICU,if the clinical symptoms of gastrointestinal dysfunction showed,it prompt poor prognosis or exacerbations.The pathogenesis of severe gastrointestinal dysfunction in modern medicine is not yet clear,and treatment also lacks specificity.The traditional Chinese medicine model is multi-target and multi way,also has the overall.concept and the charac-teristics of treatment based on syndrome differentiation.FU QI being ob-structed is the common syndrome of gastrointestinal dysfunction.Tiaowei-chengqi Decoction comes from Treatise on Febrile Diseases inYangming disease pulse syndromes and treatment of Yangming syndrome report.It has the effect of purging heat、harmonizing stomach、moistening dryness and softening hardness,and is commonly used in the gastrointestinal Hot Syndrome of Yangming disease.At present,there are relatively few studies on the treatment of severe gastrointestinal dysfunction in Tiaoweichengqi Decoction.Objective:By observing the effect.of patients with severe gastrointestinal dys-function who were treated by the Western medicine treatment combined with tiaoweichengqi decoction,to prove that tiaoweichengqi decoction can reduce intra-abdominal pressure effectively,promote gastrointestinal peristal-sis,improve oxygenation and the prognosis of the disease,and provide a scientific basis for clinical application of Tiaowei Chengqi Decoction.Methods:Apply a randomized.controlled,prospective design.50 patients were se-lected from May 2016 to March 2017 in the First Affiliated Hospital of Guangzhou University of Chinese Medicine in ICU who meet the experimental stan-dards.According to the method of randomized,patients were divided into treatment group and control group,each group was 25 cases.All the patients were given conventional Western treatment.On the basis of routine treat-ment,the treatment group was added with tiaoweichengqi decoction,reatment course was 5 days,and observed the changes of intra-abdominal pressure、abdominal circumference、bowel sounds、oxygen index and APACHEⅡ score between before treatment and after treatment.The data was analyzed with spss20.0 statistical software.Results:1.Before entering the group,all of the gender,age,intra-abdominal pressure,bdominal circumference,bowel sounds,oxygen index and APACHE Ⅱ score of between treatment group and control group is p>0.05,indicating that the two groups were comparable.2.Tiaoweichengqi decoction can improve patients with severe gastro-intestinal dysfunction.The treatment group after treatment of intra-abdominal pressure,abdominal circumference and bowel sounds were significantly better than the control group,P<0.05,there was statistical difference between the two groups.3.Tiaoweichengqi decoction can improve the patient’s oxygenation.The treatment group oxygenation index increased more than the control group,P<0.05,there was statistical difference between the two groups.4.Tiaoweichengqi decoction can improve the severity of patient’s condition.The treatment group compared with control group,APACHEⅡ score decreased significantly,P<0.05,there was statistical difference between the two groups.Conclusion:Tiaoweichengqi decoction can promote gastrointestinal peristalsis,reduce intra-abdominal pressure and abdominal circumference,increase the number of bowel sounds,improve the status of systemic oxygenation and severity acquire ideal effect in treatment of severe gastrointestinal dysfunction.
