But due to the early diabetic nephropathy often has no obvious characteristic and is easy to overlook, once diabetic nephropathy apppeares a large number of persistent proteinuria that will progress so fast that the existing medical technology is difficult to reverse. Therefore, it is extremely important for clinicians to grasp diabetic nephropathy’s comprehensive treatment and ensure early discovery and early diagnosis. Many evidence-based medical evidences have confirmed that high quality of low-protein diet and controlling such as "three highs" state are the basis and main treatment method of diabetic nephropathy now.But how to prevent the happening of the diabetic nephropathy, delay its progress and protect renal function is already the world’s problems.Abundant clinical data of studies verified that traditional Chinese medicine had extremely clinical effect in preventing early stage, improving clinical Chinese symptoms and laboratory indicators, holding back the progress of diabetic nephropathy, protecting remains renal function. As a result, the combination with traditional Chinese and western medicine’s treatment of diabetic nephropathy has its unique advantages and important clinical value.Residents are living for a long time in hot and moist climate condition of Ling-nan area, while most diabetes tend to be overweight and prefer to taste greasy food. So it could easily lead to retention of water-dampness and producing evil heat, damp-heat lastingly brewing into poison. At last, damp-heat toxin damaging renal collaterals will give rise to the diabetic nephropathy belonged to the deficient qi and yin combined damp-heat with exuberance of virulence.ObjectiveThis research adoptes the prospective study method to discusse the clinical guiding significance and curative effect on III, IV stage of diabetic nephropathy treated with the method of enhancing qi and yin, purging damp-heat and remained toxicity. At the same time, this research can enlarge the treating scope of proprietary Chinese medicine, provide a new thought and method on treating diabetic nephropathy, which is treated with Shenyankangfu tablet that has the effect of enhancing qi and yin, purging damp-heat and remained-toxicity and often used in the treatment of chronic glomerulonephritis, IgA nephropathy.MethodsThis study collected 80 cases of III, IV stage of diabetic nephropathy whose type of syndrome belonged to the deficient qi and yin combined damp-heat with exuberance of virulence. According to the odd-even sequence to hospitalize, patients were randomly divided into the two group taken in trial group and matched group. Both groups of patients chose the foundation therapeutic schemes of controling blood glucose, reducing blood pressure and regulating lipid according to the condition needs. The trial group was based on the above treatment added with Shenyankangfu tablet that had the curative effect of enhancing qi and yin, purging damp-heat and remained-toxicity. After 2 months’course, assessment of comprehensive effect, Chinese medicine symptom integral and the change of the laboratory indexes were carried out.ResultsUnder the good balance condition of baseline data and using different statistical methods, overall response rate of the comprehensive effect in the trial group was 89%, which was better than that of matched group (78%) and the trial group had obviously significant difference in improving patients’clinical symptoms and laboratory indicators of the overall condition. After treatment two groups of TCM syndrome total score were significantly decreased, but the trial group to improve TCM syndrome total integral effect was better (P<0.05), especially for five aspects of traditional Chinese medicine clinical symptoms improved significantly, the difference is statistically significant (P<0.05).UACR and UALB, HCY and Cys C were improved meaningfully by self-comparison in both groups, but the curative effect of the trial group was more outstanding in reducing urinary albumin, improving renal damage (P<0.05). In the two groups, from start to finish treatment the situation of BIM, blood sugar and blood lipid were declined, but after treatment the effect of the trial group in fasting blood glucose, glyeosylated hemoglobin were more significantly improved than matched group(P<0.05).ConclusionIt had obvious effect to improve TCM symptoms, lower urinary albumin, delay renal damage, protect kidney and ameliorate glycometabolic disorder treated with the method of enhancing qi and yin, purging damp-heat and remained-toxicity for Ⅲ, Ⅳ stage of diabetic nephropathy whose type of syndrome belonged to the deficient qi and yin combined damp-heat with exuberance of virulence.
