The Study On Integration Of Maternal Health Care

ObjectivesBased on the theoretical research,we refine the key elements of the integration of health care services and analyse the relationship between the elements preliminarily.In addition, adjust the Maternal service integration elements again by analyzing the characteristics of the Maternal Health Care provided in the maternal and child care service centre and family planning service center and field research, in order to provide some references for promoting integration under the background of integration of maternal and child health and family planning service system.ContentsFirstly, we refine the key elements of the integration of health care services by sorting the theory and practice of the integration of health both at home and abroad.Sencondly, analyze the characteristics of the Maternal Health Care provided in the maternal and child care service centre and family planning service center.Thirdly, investigate sample areas to understand the the status quo of the integration and improve the elements of integration.Lastly, provide some references for promoting integration of maternal health care.MethodsThis study used the literature research method combing the theory and practice of the integration of health both at home and abroad. Experts’advices about the research scheme as well as the scientific,conduct and suitability of research plan and tool are also important to this article, and then adjustment should be implemented on the basis of experts’opinions.Collect qualitative data by the individual in-depth interviews and focus group interview of health administrative personnel, maternal and child and family planning institutions, health service personnel and patients and collect quantitative data through the questionnaire.This study used descriptive statistical analysis, inductive method and qualitative research method to analyze the material. The study used descriptive statistical analysis method to analyse the satisfaction of the staff and patients of maternal service.Nvivo8.0 software was used to establish coding and node of key point in qualitative interview and output model after summarize the various nodes.Results1. Based on the theoretical framework of health services integration,the seven key elements of integration were summarized which included policy support, understanding and consensus, leadership, financing and compensation mechanism, human resource integration, service coordination, information technology. Human resource integration is mentioned to the most,followed by information technology, policy support, financing and compensation mechanism. Leadership, service coordination, understanding and consensus was mentioned less.In addition, each element was not isolated and they influenced each other.2. In China,the maternal health care is provided both by the maternal and child health and family planning service agencies.Their financing mode, personnel qualifications, the style of leadership are different and they are similar in the service content,information systems. In the future,we should focus on the different aspects,properly handle contradictions and avoid damaging the interests of any part. At the same time,the repeated service should be merged to save resources and improve efficiency.3. In terms of maternal health care integration both in Hubei and Qinghai Province,they established leadership and policy support mechanism,implemented the organizational merge rapidly, adjusted the service contents and methods reasonably and integrates optimized the service content and process and improved service delivery, maintained the original compensation mode unchanged, followed the principle of "no reducing headcounts and no downsizing" strictly in order to ensure the team stability and motivation, integrated the maternal and child and family planning information platform.The integration in both province was progressing and the satisfaction of staff and service object was high,however,there were also some difficulties,such as staffing issues. Besides,this study used the qualitative analysis method to analyze the qualitative data, did statistics the frequency of key elements,and output the mode which include human resource integration, policy system, the understanding of the integration, financing and compensation mechanism, service contents and service process, information system.ConclusionsThis study held that the maternal health care integration including integration of policy system, the understanding of integration, the service content and form, human resource integration, financing and compensation mode integration, information supporting. The correct understanding of integration can promote the enthusiasm of participants.Policy system support human resource integration,financing,information system. The integration of service content and form require the support of policy system and financing and this aspect of the integration needs to advance further.Human resources integration is the most important element of integration and the key step of the whole process which could affect implementation process.Financing and compensation mechanism provides guarantee funds for service integration. Build a unified maternal and child and family planning information system has become the inevitable choice of health care integration.RecommendationsTo strengthen publicity of the importance of maternal health care integration by using a variety of propaganda means; breakthrough the limitation of the original service content and service delivery form,guided by the demands of the service, to optimize service content and enhanced service initiative; breakthrough the limitation of identity,to establish a new mechanism of choosing and employing persons and should build a system of full employment, through competition; on the premise of integration work smoothly, to innovate the compensation mode and establish the long-term mechanism; attacheing great importance to the information systems function and increasing investment in information system,to improve the capacity of information technology and integrate the information system in the maternal and child care service centre and family planning service center.
