Research On The Collaborative Mode Of Health Service Delivery In Three-level Maternal And Child Health Care Institutions Oriented By "Integration"

The domestic medical service system is now faced up with the dilemma of "segmentation" and"fragmentation",besides,"chain-broken" and "fragmented" heath service has multiple problems regarding management system,service system and institutional mechanism,which could not effectively guide patients to seek medical treatment at different levels,but also reduces the overall efficiency and quality of medical service Therefore,heavy obstacles in joint construction of health service have become the bottlenecks urgent to be solved in health care reform.The fertility policy has been adjusted twice in last three consecutive years,in order to relieve economic pressure brought by increased aging,as well as to stimulate the willingness of the people.However,fertility rate declined rather than rose after the full liberalization of the two-child policy.According to statistics,the proportion of people having two children has increased,but the first-child fertility rate has been continuously decreasing,the expected "compensatory baby boom" has not appeared yet.The three-level diagnosis and treatment system in maternal and child health care institution has been initially established in our country,but it responses slowly to rapidly growing population fertility needs,especially for grassroots maternal and child health care institutions which have absolute advantage in quantity.The paper is divided into ten chapters based on systematic characteristics of research contents,the first chapter briefly describes current research status of domestic and international health service provision,the second explains related concepts and applications from the theoretical perspective,the third introduces main research contents and methods,the fourth traces historical development and evolution course of maternal and child health care institutions,the fifth compares operating mechanisms of different kinds of maternal and child health care institutions,the sixth refines characteristics and influence factors of maternal and child health care service provision,the seventh discusses development status of domestic medical union,the eighth analyzes system causes for the lacking collaborative system of health provision in three-level maternal and child health care institutions,and puts forward the implement approach of health provision for maternal and child health care institutions under the guidance of "integration" philosophy,which is in line with China's national conditions,in addition,the ninth and tenth chapters point out the innovation and insufficient,summary and prospect of the paper,respectively.The paper has widely collected domestic and foreign articles regarding health service provision and collaboration of maternal and child health care institutions for further systematic review,summary and analysis,as well as stakeholder dimension analysis,with the aim to approve grassroots maternal and child health care service system,provide equalized health care for women,strengthen the fertility willingness of the people,and optimize population structure.Based on the realistic dilemma of huge economic and social development pressure brought by increased population aging,the research puts forward the strategy set beneficial for demographic adjustment,which could provide theoretical basis for government to formulate social and economic policies that are more in line with China's national conditions.The paper not only summarizes international development situation of macroscopic medical union,but also deeply describes collaborative mode of our national typical health service provision,besides,it takes demographic adjustment as an entry point to elaborate current status,missing nodes and optimization paths of health service provision of three-level maternal and child health care institutions in our country.Finally,the paper proposes the research mode of "integration" oriented health service provision of three-level maternal and child health care institutions,and finds certain deficiency through theoretical and empirical research,which needs to be continuously improved in further research to ensure more accurate and practical research conclusions.The paper conducts survey and collection of abovementioned evaluation data based on the three-dimensional health project evaluation framework of Donabedian structure-process-result,and evaluates high-quality and low-quality indicators through Topsis analytical method and normalized raw data matrix.Moreover,the paper performs assimilation analysis of all indicators to determine actual collaborative effect,or distance between pending collaborative strategy and ideal solution and negative ideal solution,and to calculate actual collaborative effect or relative proximity between pending collaborative strategy and ideal solution.In the end,the paper performs comprehensive comparative analysis of missing collaborative degree of health service provision in three-level maternal and child health care institutions through calculation results,screens key missing nodes and presents the collaborative optimization list,and schemes out the strategy set beneficial for demographic adjustment,which could provide theoretical basis for government to formulate social and economic policies that are more in line with China's national conditions.
