Objective: To observe the width and the changing characteristics of the pedicle in osteoporotic vertebral compression fractures patients. Methods: Osteoporotic vertebral compression fracture(OVCF) patients over 60 years old who were admitted to Spine Surgery Department, the Affiliated Hospital of Sichuan medical university and underwent percutaneous kyphoplasty(PKP) from June 2012 to January 2015 were selected as the group A, while thoracolumbar vertebral fracture patients below 50 years old were enrolled as the group B according to inclusion and exclusion criteria. All patients received fracture site-centered CT plain scanning and 3D reconstruction imaging. Image post-processing ADW4.4 Workstation was employed to observe the pedicle out widths(POW) corresponding to T11~L5 in both groups by utilizing multiplanar reconstruction(MPR) technique. Pedicle widths in both groups, POW of males and females in both groups, and the percentages of the POW less than 5 and 7mm in both groups were compared respectively. Results: There were 152 patients in Group A, including 35 male and 117 female. The mean age was 72.5(range 60~95) years old. There were 2128 pedicles in Group A. Fracture vertebras were located in:T11 18 cases, T12 36 cases, L1 63 cases, L2 33 cases, L3 20 cases, L4 12 cases, L5 5 cases. 86 cases were single spine fractures, and two or more 66 cases. The mean POW of T11~L5 were: 6.2mm, 6.6mm, 5.6mm, 6.1mm, 7.1mm, 10.1mm, 12.5mm,while in the male were:6.6mm, 7.0mm, 6.1mm, 6.7mm, 7.6mm, 11.4mm, 13.2mm,and in the female were: 6.1mm, 6.5mm, 5.4mm, 6.0mm, 7.0 ± 1.3mm, 9.7mm, 12.2mm. There were 192 patients in Group B, including 127 male and 65 female. The mean age was 33.5(range 26-50) years old. There were 2688 pedicles in Group B. The mean POW of T11~L5 were: 7.7mm, 8.3mm, 7.6mm, 8.0mm, 8.5mm, 10.4mm, 12.8mm,while in the male were: 7.9mm, 8.4mm, 7.7mm, 8.1mm, 8.7mm, 11.0mm, 13.0mm,and in the female were: 7.3mm, 8.1mm, 7.3mm, 7.7mm, 8.2mm, 9.4mm, 12.4mm. The statistical results showed that there were no statistically significant differences between POW of the left and right sides in both groups(P>0.05); the POW in male were significantly bigger than that of female in all T11~L5 of both groups(P<0.05); POW corresponding to T11~L5 in the group A were 1.5mm, 1.7mm, 2.0mm, 1.9mm, 1.4mm, 0.3mm and 0.3mm smaller than those in the group B. The POW of T11~L3 in the group A significantly were smaller than Group B(P<0.01),while no statistically significant differences of L4 and L5 between both groups(P>0.05). The percentages of POW less than 5mm in male and female of group A were 4.9% and 12.1%, while the percentages of POW less than 7mm in male and female of group A were 0.4% and 1.5%. The ratios of POW less than 5mm and 7mm corresponding to T11~L5 in the group A were higher than Group B,especially in the women of group A, mainly distributed in T11~L3. Conclusions: 1. POW corresponding to T11~L3 of elderly OVCF patients are generally smaller than young and middle-aged patients,especially the POW of L1 and L2; 2. POWcorresponding to T11~L5 of elderly OVCF patients of male are generally bigger than female; 3. The percentages of POW less than 5mm and 7mm in old male OVCF patients are 4.9% and 39.0%,while in female are 12.1%、53.8%,both mainly distributed in T11~L3.
