Objective: To study joint effect and the best action site of medical ozone and methylprednisolone on radicular sciatica.Methods: 120 cases of L4/5 or(and) L5/S1 lumbar disc herniation with radicular sciatica diagnosised by radio-clinical manifestation.According to the injection site, All of the patients were randomly divided into four observation groups: Lateral recess Group(Group L), the intervertebral foramen Group(Group I), the lateral recess and sciatic gluteal point Group(Group LS),the intervertebral foramina and Sciatic gluteal point Group(Group IS), 30 patients in each group. Ozone(5ml, 30μg/ml) and methylprednisolone(20mg) were injected respectively at each site once per week, three times in a row. To Record the numerical rating scales(NRS) scores and the Oswestry disability index(ODI) scores and the treatment-related adverse reactions at the following time points,Before the treatment(T0),1 week after the first treatment(T1),1 week after the second treatment(T2),1 week after the third treatment(T3),1 month after three treatments(T4), 3 month after three treatments(T5),6 month after three treatments(T6).Results:1. NRS/ODI scores: There were different trends of variation at different time-point in 4 groups before and after treatment, The decreasing range of NRS/ODI scores in Group LS and group L were more than that of group I and group IS, showed that a trend which decreased at first and then slightly increasing. Compared with T0, the NRS/ODI scores of 4 groups were significantly decreased after treatment at each time-point(P<0.05). Compared with group I and group IS, NRS/ODI scores of group LS and group L were significantly decreased(P<0.05) at T3, T4, T5 and T6 time-point. There were no significant difference(P>0.05) between group I and group IS in NRS/ODI scores, and neither no statistical difference(P>0.05) between group LS and group L.2. Adverse reaction: 1 case in lateral recess Group was found to have heart palpitations and sweating at the first treatment. There was no adverse reation observered in other groups. No complications were noted in all of the 4 groups after treatment. There was no statistically significant difference(P>0.05) on adverse reation.Conclusions: To use medical ozone and methylprednisolone injection for the treatment on radicular sciatica is safeand effective. The treatment action by using lateral recess, lateral recess and sciatic nerve as the injection site will be more effect than other sites, lateral recess may be the best injection site, and the operation is simple and convenient.
