Esophageal Cancer Patients With Different Surgical Methods On The Influence Of Humoral And Cellular Immune Function After Surgery

Ojcetive This study aims to comparate the changes and relationships of immunity index of esophageal cancer patients under videl-assisted thoracospcopic surgery (VATS) and routine operation. From the Angle of the immune mechanism of objective analysis advantage of thoracoscope surgery in the treatment of esophageal cancer.Methods Retrospective analysis of January 2014 March-September 2014 at the Frist Affiliated Hospital of Anhui Medical University,51 randomized extraction esophageal cancer patients were classified into VATS group (n=16) and routine opertion group(n=35). The level of IgA, IgM, IgG、C3、C4、CRP and the content of T-lymphocyte subgroups (CD3+, CD4+、CD8+)and　NK cells were measured by the blood sample which was taken from these two groups at the following time:day before operation, the first postoperative day and the sixthe postoperative day.Results 1.Traditional group and VATS group of patients with preoperative immune globulin IgA, IgG, IgM and CRP level is no statistical difference (P>0.05).At the first postopertive day, the level of IgA、IgM、IgG、CRP in VATS group is lower than the index in routine operation group (P< 0.05).But immunoglobulin levels between traditional group and VATS group had no statistical difference(p>0.05)..At the sixth postoperative day, two groups of immunoglobulin IgA, IgG, IgM and CRP levels are 1 d after all was increased, but still below the preoperative level(P<0.05).And compared with the traditional group,VATS group IgA, IgG, IgM and CRP levels increased significantly, the difference was statistically significant (P<0.05).Postoperative 1 d, traditional group and VATS group of C3 and C4 level compared with preoperative has no obvious change(P>0.05).Immunoglobulins leels between traditional group and VATS group were no statistical differences.6 d, two groups of C3 and C4 level compared with preoperative and postoperative 1 d had no obvious change(P>0.05),And compared with the traditional group, there was no statistically significant difference VATS group((P>0.05)). 2.Preoperative T lymphocyte subsets of patients with two groups of CD3+, CD4+, CD8+, CD4+/ CD8+ and NK cells content differences had no statistical significance (P> 0.05).Preoperative T lymphocyte subsets of patients with two groups of CD3+, CD4+, CD8+, CD4+ / CD8+ and NK cells content differences had no statistical significance (P> 0.05).But it had no statistical significance between the two groups of T lymphocyte subsets and NK cells content differences.But between the two groups of T lymphocyte subsets and NK cells content differences had no statistical significance.VATS group of CD3+, CD4+, CD4+ / CD8+ and NK cells content level increased,6 d CD3+ and NK cell levels there was no statistically significant difference compared with before operation (P> 0.05), and CD8+ cells levels is slightly lower than 1 d postoperatively (P< 0.05), has been basically close to preoperative levels (P> 0.05). Comparison between the two groups of various testing data can be obtained,6 d, VATS group of CD3+, CD4+, CD4+ / CD8+ and NK cell levels were higher than conventional group (P<0.05), and CD8+ cells levels lower than the traditional group (P<0.05)Conclusion 1.Patients with esophageal cancer patients with surgical trauma for both cellular immunity and humoral immunity inhibition, smaller influence on complement.2.Thoracoscope surgery in patients with esophageal small trauma, impact on the postoperative patients’ immune level is small, compared with traditional radical after illness recovery is superior to the traditional radical prostatectomy.
