Significance Of Sonography Automated Breast Volume Scanner In The Preoperative Assessment Of Breast Lesions

Objective: Automatic breast volume scanner(ABVS) and handheld ultrasonography were used to measure breast lesions confirmed by pathology, comparing these two methods in diagnosis of benign and malignant breast lesions, and assess the value of ABVS in the preoperative diagnosis of breast lesions.Mothed: From January 2014 to December 2014, a total of 172 patients(18 to 74 years old, average 44.60±12.98) with 191 breast lesions were examined by HHUS and ABVS measuring the maximum diameter of transverse, coronal and sagittal plane. To pathology results for the gold standard, the relevant indicators has been calculated.Result:Of all the breast lesions,there were 87 benign lesions and 104 malignant lesions, including 94 cases of invasive carcinoma and 10 noninvasive carcinoma.ABVS and HHUS had no obvious differences on breast benign lesions measurements(P>0.05). Compared with the pathological results, ABVS was superior to HHUS on malignant lesions measurements(P<0.05).For noninvasive lesions(including ductal carcinoma and mucinous carcinoma), ABVS was superior to HHUS in Y-axis and Z-axis measurements(P < 0.05), but in X-axis, it has no obvious differences(P>0.05).Lesions in invasive group(including the infiltrating ductal carcinoma, invasive lobular carcinoma and sarcomatoid carcinoma) ABVS measured value superior to HHUS measuring values(P<0.05).In cases that the maximum diameter of pathological measurement were more than 5.0cm, ABVS is superior to HHUS in X-axis, but in Y-axis and Z-axis, it has no obvious differences(P>0.05).And for lesions pathologic maximum diameter less than 5.0 cm, measurement ABVS HHUS measured value is obviously better than the measured value(P < 0.01), shows a survey of small lesionsIn the evaluation of benign lesions, the overall accuracy,specificity, sensitivity, positive predictive value(PPV) and negative predictive value(NPV) are 77.8%、70.1%、87.5%、77.8%、82.4% using HHUS and are 98.7%、99.0%、87.4%、98.7%、90.4% using ABVS, respectively.In the evaluation of malignant lesions, the overall accuracy,specificity, sensitivity, positive predictive value(PPV) and negative predictive value(NPV) are 77.8%、70.1%、87.5%、77.8%、82.4% using HHUS and are98.7%、99.0%、87.4%、98.7%、90.4% using ABVS, respectively.Conclusion : ABVS measurement is better than that of HHUS of malignant breast lesions, particularly for noninvasive lesions and the larger maximum diameter lesions have certain advantages. Its unique surgical perspective can provide more accurate and intuitive information for confirming the size of lesions during the surgery and for surgical plan making.
