Objective: To explore the application value of automated breast volume scanning(ABVS)and virtual touch tissue quantification(VTQ)in the differential diagnosis of breast benign and malignant lesions.Method: A total of 164 breast masses were selected from 136 patients treated in our hospital from October 2019 to June 2020.All breast masses were confirmed pathologically after surgery or biopsy.Use Siemens ACOUSON S2000 ultrasound instrument equipped with ABVS imaging system,9L4 high frequency probe with frequency of 4MHz?9MHz and acoustic palpation quantization technology software;obtain conventional ultrasound images of all masses,ABVS coronal image and VTQ shear wave velocity Value(shear wave velocity,SWV),with pathological results as the gold standard,analyze the sensitivity,specificity,accuracy,positive predictive value and positive predictive value of conventional ultrasound,ABVS,VTQ technology and ABVS combined with VTQ technology in the diagnosis of benign and malignant breast masses Negative predictive value,the receiver operator characteristic curve(ROC)of each inspection method was constructed,the best diagnostic threshold of VTQ technology SWV was obtained,and the area under the curve of each inspection method(area under the curve,AUC),to compare and analyze the diagnostic value of various examination methods for breast masses.Result: 1.164 breast masses,75 of which were malignant by pathology and 89 were benign by pathology.2.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of conventional ultrasound in the diagnosis of benign and malignant breast masses were 64%,89.89%,78.05%,84.21%,74.77%,and the AUC was 0.769.3.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of ABVS in diagnosing benign and malignant breast masses were 73.33%,95.51%,85.37%,93.22%,80.95%,and AUC was 0.844.4.The mean SWV values of VTQ technique for benign and malignant masses were 2.55±1.31m/s and 6.07±2.68 m/s,respectively,and the difference between the two was statistically significant(P <0.05).The SWV values of all masses were constructed into ROC curves.When the Youden index reached the maximum of 0.6526,the corresponding cut-off point was 3.76 m/s,and the 95% CI was 0.784-0.900,that is,when the SWV diagnostic threshold of VTQ technology was 3.76 m/s,The diagnostic efficiency of benign and malignant breast masses is the best.Its diagnostic sensitivity,specificity,accuracy,positive predictive value,negative predictive value are 72%,93.26%,83.54%,90%,79.81%,and the AUC is 0.826.5.The sensitivity,specificity,accuracy,positive predictive value,and negative predictive value of the combined application of ABVS and VTQ technology in the diagnosis of benign and malignant breast masses were 90.67%,89.89%,90.24%,88.31%,91.95%,and AUC was 0.903.6.Comparison of diagnostic efficiency of different examination methods: The combined diagnostic sensitivity and accuracy of ABVS and VTQ technology are significantly higher than that of conventional ultrasound,and the difference is statistically significant(P<0.05);the combined diagnostic sensitivity of ABVS and VTQ technology is significantly higher than that of ABVS and VTQ alone Application,the difference is statistically significant(P<0.05);ABVS has the highest diagnostic specificity,but the difference is not statistically significant(P>0.05).The area under the curve of combined diagnosis of ABVS and VTQ technology was significantly larger than that of conventional ultrasound,ABVS,VTQ,and the difference was statistically significant(P<0.05).The area under the curve of ABVS was significantly larger than that of conventional ultrasound,and the difference was statistically significant(P<0.05).Conclusion 1.Compared with conventional ultrasound,ABVS and VTQ have higher diagnostic value for breast masses.The high specificity of ABVS coronal surface provides more reference for the diagnosis of breast cancer.The SWV value of VTQ technique is significantly different between benign and malignant breast masses.When the SWV diagnostic cut-off value is 3.76 m/s,the diagnostic efficiency of benign and malignant breast masses is the best.2.The combined application of ABVS and VTQ can improve the diagnostic sensitivity and accuracy,and the combined application value is higher than that of conventional ultrasound,ABVS and VTQ alone.
