Objective : To investigate the health service utilization among undergraduates in a university of Chinese People’s Armed Police Forces(PAPF) and analyse the existing problems, thus providing reasonable countermeasures and suggestions pertinent to the improvement of the college’s health service utilization.Methods:In reference to the relevant survey items of the health service utilization in Chinese Health Service Survey in 2008 and combined with the characteristics of the respondents at the same time, the questionnaire on health service utilization among certain PAPF college students was designed. The survey was conducted among 1908 undergraduates including all of the 1-4 grade students in a PAPF comprehensive college. The study contents include respondents’ basic situation, basic health condition, health service utilization, health service satisfaction and health service demand.Results:1 General information: There were 1413 male students and 495 females, which accounted for 74.06% and 25.94% respectively. The age of the students ranged from 18 to 26 years old, and the students aged from 21 to 23 were the majority(52.41%), 1833(96.07%) students came from the Han nationality and 75(3.93%) students came from the minority ethnic groups,866(45.39%) students were the only child in the family and 1042(54.61%) students were not, 1064(55.77%) students majored in medical science and 844(44.23%)students majored in other disciplines; There were 1056 people whose family income was more than 3000 yuan per month, accounting for 55.35%; 63.68% of the college undergraduates came from cities and towns, and freshmen made up the majority, accounting for 30.60%.2 Basic health condition: 1356 students(71.07%) evaluated their own health condition as good, 484(25.37%) regarded theirs to be average, and 68(3.56%) thought of theirs to be poor. By investigating such health-related conditions as diet, exercises, smoking, sleep, stress, and so on, the results showed that 94.02% of respondents could ensure to exercise for more than three times a week, 82.39% spent more than 30 minutes on physical exercises each time, 81.55% had not been smoking currently. Bad health behaviors held by respondents mainly included sleep time of less than 7 hours each day(60.48%) and the overwhelming psychological pressure(43.92%).3 Health service need: Within two weeks, the number of sick people was 546, with 740 person-time cases and the two-week prevalence rate was 38.78%, which was higher than that of the national average level(24.10%)(P < 0.05). The statistical results of two-week prevalence rate analyzed according to different demographic characteristics revealed that two-week prevalence rate in males(34.25%) was significantly lower than that in females(51.72%)(P<0.05), and that of self-evaluated good health status(31.56%) was significantly lower than that of self-evaluated bad health status(85.29%)(P< 0.05), but the prevalence rate of first-grade students(45.38%) was significantly higher than the other grade ones(P<0.05), and that of non-medical students(47.87%) was also significantly higher than that of the medical ones(31.58%)(P<0.05), however, there was no statistical significance in the prevalence rate of students with such conditions as only-child condition, urban or rural condition, or family income(P>0.05). Logistic regression analysis showed that the gender, major and grade were the main influencing factors that caused the illness among college students.4 Health services utilization:(1) Consultation rate of respondents within two weeks was 29.56%, which was higher than that of the national average level(13.00%)(P<0.05). The consultation rate of two weeks were analyzed according to different demographic characteristics, and the results showed that the two-week consultation rate in males(25.76%) was significantly lower than that in females(40.40%)(P<0.05), with the rate of the fourth-grade students(28.13%) lower than that of the other grade students(P<0.05), the rate of medical students(24.44%) lower than that of the non-medical students(36.02%)(P<0.05), and the rate of self-evaluated good health status(25.88%) lower than that of self-evaluated bad health status(70.59%)(P<0.05); however, there was no statistical significance in the consultation rate of students with different conditions, such as the only-child condition, urban or rural condition, or family income(P> 0.05).(2) 76.22% sick respondents went to see the doctor, but 23.78% didn’t within this period of time because they thought they were not serious enough to go to see the doctor, they could take non-prescription drugs by themselves, or they didn’t have enough time.(3) Annual hospitalization rate was 3.72%, the average time span for hospitalization was 6.20 days, and the rate was 42.28% for patients who needed hospitalization and had not been hospitalized, with the same reasons mentioned above of the 42.28% patients who didn’t go to see the doctor. Logistic regression analysis showed that the gender, major and grade were the main influencing factors of two-week consultation rate.5 Health service satisfaction and demand: 75.84% of the respondents felt satisfied with the college’s overall existing health service support modes, 45.39% felt satisfied with the health service work and 43.97% had a general feeling towards the service, 10.64% unsatisfied. The number of students who were satisfied with health education of the college accounted for 53.41%. The most satisfactory service was the transfer work in clinic, however, drug variety, humble medical equipment, and poor medical skill of medical staff needed promoting. The most pressing need was out-patient medical service in the college, followed by preventive health care and psychological counseling.Conclusion:Two-week prevalence rate of the respondents was high, and the gender, major and grade were the main factors that caused the illness among college students. Health service was in great need among students who had some adverse health behaviors such as under great pressure and lack of sleep. Two-week consultation rate of the respondents was high, as the gender, major and grade were the major influencing factors of the consultation rate. Low rate of annual hospitalization of college students, after the sick not go to see the doctor and needed hospitalization and had not been hospitalized rates on the high side should be hospitalized. The survey showed that there was still a gap between the overall health service ability and the expectation of the students in the college.
