Objective : To investigate the level of therapeutic adherence of the elderly hypertensive patients in one region of countryside, and analyze the correlation factors that affecting the patients therapeutic adherence,so as to select focus intervention groups. In order to provide reference for carrying out health education and make specific intervention and improve patients health. Methods:Elderly with hypertension had been selected from some countrysides in one town, one province. For each participant, the demographic data were colle cted from the registration documents at the community health service station.Th en face to face interview was performed to complete a questionnaire. The rese archer used instruments included general demographic and nosography informati on of respondents,Therapeutic Adherence Scale for Hypertensive Patients,Anti-hy pertensive Therapy Related Attitudes and Beliefs Scale,Family concern index qu estionnaire,Chinese Medical Outcomes Study Social Support Survey.General dem ographic and nosography data used descriptive statistical analysis, comparison of two groups of mean used t test and multiple groups used analysis of variance analysis, multiple factors analysis used multiple regression analysis, using Pearso n correlation analysis of variables, conducted by SPSS17.0.Results:1. The scores of therapeutic adherence of the elderly hypertensive pati ents in countryside was(89.06±10.33),71.25% of their scores. Each dimension scores:behavior of medication adherence mean 18.59 scores, behavior of medicati on non-adherence mean 30.42 scores, behavior of daily life management mean 31.77 scores, behavior of smoking and drinking mean 8.28 scores.2. Single factors variance analysis results showed that BP controller succeed,in high age, have medical staff in the home, have hypertension complications or complications,self-evaluated well effect of anti-hypertensive treatment, the elderly h ypertensive patients in countryside have high scores in the total scores and eac h dimension, differences are statistically significant(P<0.01 or P<0.05);have fa mily history of hypertension,long-course of administration anti-hypertensive med ications, the time consult doctor frequently, the elderly hypertensive patients in co untryside have high scores in the total scores and in behavior of medication ad herence, behavior of medication non-adherence,behavior of daily life management, differences are statistically significant(P<0.01 or P<0.05);the long-course of h ypertension, the elderly hypertensive patients in countryside have high scores in the total scores and in behavior of medication adherence, behavior of medication non-adherence,differences are statistically significant(P<0.01 or P<0.05);self-e valuated well effect of anti-hypertensive treatment, the elderly hypertensive patie nts in countryside have high scores in the total scores and in the behavior of medication non-adherence and behavior of smoking and drinking, differences are statistically significant(P<0.05);male have higher scores in the behavior of dai ly life management than female, but female have higher scores in the behavior of smoking and drinking(P<0.05).3. A multiple regression analysis showed that:age,BP controlling status, whether medical staff in the home, family history of hypertension, hypertension complications or complications,self-evaluated effect of anti-hypertensive treatment, the time consult doctor, are the main factors that affect the level of the therapeutic adherence in elderly hypertensive patients from countryside with joint explained 36.3% variation.4. Correlation analysis showed that anti-hypertensive therapy related attitudes and beliefs, family function and medical outcomes study social support is positively related with the total score of the therapeutic adherence and behavior of medication adherence, behavior of medication non-adherence,behavior of daily life management, correlation index is(0.358~0.592 & 0.126~0.361 & 0.297~0.395,P<0.01),family function is negative related with behavior of smoking and drinking(r=-0.128,P <0.05).Conclusion : 1.The therapeutic adherence of the elderly hypertensive patients in countryside are in the low level.2. Age,BP controlling status, whether medical staff in the home, family history of hypertension, hypertension complications or complications,self-evaluated effect of anti-hypertensive treatment, the time consult doctor, are the main factors that affect the level of the therapeutic adherence in elderly hypertensive patients from countryside.3. The patients who are in low age,BP controlled failure,no medical staff in the home, have family history of hypertension, have hypertension complications or complications,self-evaluated poor effect of anti-hypertensive treatment, few time consult doctor, can be selected for the emphasized intervention groups.4. The therapeutic adherence in elderly hypertensive patients from countryside are closely related with anti-hypertensive therapy related attitudes and beliefs, family function, medical Outcomes Study Social Support, guide patients change attitudes and beliefs、strengthen medical social support and family function, can improve the level of the therapeutic adherence.
