Objective1. To improve the compliance of early activity among patients with high risk of deep vein thrombosis. 2. To evaluate the efficacy of the action research with reducing the incidence of DVT and promoting the joint function in a better way. 3. To spread the program of early mobilization in the department with high risk of deep vein thrombosis, such as orthopaedics department, oncology department, etc.MethodsQuasi-experiment design was used in this study. The action research group and the control group was divided according to the time of admitted to hospital. Routine nursing care was used in the control group, action research was used to guide the action research group through daily observation perioperative beside bed, seek out the reasons of low compliance in early activity and find out methods to solve the problems, then applied. Three sequential action research cycles built upon baseline data including 45 inpatients. The general information of participants, the compliance of early mobilization, the incidence of deep vein thrombosis, the score of hip joint function scale preoperative and related data was recorded and analyzed by SPSS 21.0. The statistic method contained descriptive statistics, independent samples t test, paired sample t test, chi-square test, two independent sample rank and two related sample rank.Results1. 92 cases were selected into the study, 2 cases were lost in the control group, 1 case was lost in the action research group, the attrition rate of the sample was 3.26%. a total of 89 cases were involved in this study finally, 44 cases in the control group and 45 cases in the action research group. 2. The independent t test and chi-square test analysis indicated that there were no statistical significance in the general demography, the score of Wells risk assessment, the score of Harris hip function scale preoperative between the two groups(P > 0.05). 3. The compliance of early activities between two groups: The independent t test and two independent sample rank analysis indicated that there was statistical significance in the compliance of early activities between the two groups(P < 0.05). 4. The incidence of deep vein thrombosis between the two groups: There were 6 patients with thrombosis in control group, no one in action research group. Chi-square test indicated that there was statistically significant between the two groups(P < 0.05). 5. The score of hip joint function scale before and after operation in control group: The paired sample t test and two related sample rank indicated that there were statistical significance in pain dimension, function dimension, dimension gait entry, deformity dimensions, mobility and the total score(P < 0.05), there was no statistical significance in the entry of the activities and functions(P > 0.05). 6. The score of hip joint function scale before and after operation in action research group: The paired sample t test and two related sample rank indicated that there were statistical significance in pain of dimension, function dimension, gait entry, the entry of the activities and functions, deformity dimensions, motion on the dimension and the total score difference had statistical significance(P < 0.05). 7. The score of hip joint function scale postoperative between two groups: The independent t test and chi-square test analysis indicated that there were statistical significance in all items except deformity dimension between two groups(P< 0.05).Conclusion1. The problems which would prevent the early mobilization can be solved by action research; 2. The incidence of DVT among patients at high risk of venous thrombosis with hip replacement can be reduced and the function of hip replacement can be improved by the high compliance of early mobilization according to the action research. 3. The effect of the program of early mobilization in the orthopaedics department is good and can be spread into other departments with high risk of deep vein thrombosis.
