Objective:To compare the effect of locking compression plate, anterograde intramedullary nails and retrograde intramedullary nails for treatment of distal femoral fracture, and to select the appropriate internal fixation method. Methods:From May2007to November2011,118patients with distal femoral fractures were treated with internal fixation in the hospital and their clinical data were analyzed retrospectively. Among them,38cases received locking compression plate,21cases received anterograde intramedullary nails, and59cases received retrograde intramedullary nails. The intraoperative blood loss, operative time, fracture healing time and rate of good postoperative recovery of knee joint in three groups were compared. Results:118patients have been followed up, time for14to26months, an average of20months. All patients without infection, except1case of patients with bone defect healing delay, caused by bone fractures with bone after filling in week19basic completely heal, healed within4.5months. Three contrast have no obvious differences in fracture healing time (p>0.05). Intraoperative blood loss and operation time, anterograde intramedullary nail group and retrograde intramedullary nail group were superior to LCP, anterograde intramedullary nail group is superior to retrograde intramedullary nailing group, difference has significant statistical difference (P<0.05). LCP, anterograde intramedullary nail group and retrograde intramedullary nail group knee Kolment grading fine rate was76.3%,52.4%and76.3%respectively. Conclusion: Retrograde intramedullary nail fixation is firm, stable and reliable, less intraoperative blood loss, shorter operation time, and the advantages of small trauma, fracture easily reset, especially in the aspect of the knee joint function recovery effect is good, compared with LCP and anterograde intramedullary nailing group, retrograde intramedullary nailing treatment of distal femoral fractures has more advantages.
