Objective To evaluate the safety and clinical value of laparoscopic radical prostatectomy in the treatment of localized prostate cancer, through comparing laparoscopic radical prostatectomy with retropubic radical prostatectomy on perioperative complications and clinical efficacv. Methods The clinical data of52patients receiving laparoscopic radical prostatectomy from January,2009to October,2013and37patients receining retropubic radical prostatectomy were retrospectively analyzed. The clinical data of these patients were gathered, including preoperative parameters, amount of bleeding during operation, operating time postoperative pain score, recovery of intestinal function after operation, timing of catheter removal, perioperative complications timing of postoperative hospital stay,one-year continence rate, positive surgical margin rate, etc. Results Two groups were performed successfully. No case was converted to open surgery in the LRP group. The mean bleeding volume during operation of the LRP group and the RRP group were (314±201) ml and (785±526) ml,and there was statistical difference between the two groups (p<0.01).The mean operative time of the LRP group and the RRP group were (236±131) minutes and (192±105) minutes (p<0.01) respectively.Additional pelvic lymph node dissection needed an average of52minutes and47minutes more, respectively. The mean postoperative pain socre, recovery time of intestinal function, timing of catheter removal, hospitalization stay after operation between LRP and RRP were (2.37±1.89) and (3.46±1.53)(p<0.05),(65±16) hours and (69±21) hours (p>0.05),(13.84±2.68) days and (14.25±3.62) days (p>0.05),(11.6±4.3) days and (14.5±6.2) days (p<0.05) respectively. And the one-year continence rate, the positive surgical margin rat after operation between LRP group and RRP group were90.3 %(28/31) and88.1%(37/42)(p>0.05),27%(10/37) and23.1%(12/52)(p>0.05),and there was no statistical difference between the LRP group and the RRP group between these indexes. Conclusion The study showed that laparoscopic radical prostatectomy is not only minimally invasive, less bleeding and faster recovery, but also provide equivalent operative-related complications compared with retropubic radical prostatectomy. Therefore,LRP is safe and reliable for the treatment of localized prostate cancer.
