Background:Heart failure is a syndrome of cardiac insufficiency caused by various heart diseases. In most cases, decreased myocardial contr-actility, cardiacoutput can not meet the demand of the body’s normal metabolism, and make the organ, tissue hypoperfusion. At the same time the pulmonary circulation and (or) the performance of circulation congestion. Due to its high mortality and hospitalizationrates, heart failure has become one of the major diseases that affecthuman health. BNP is a peptide synthesis and secretion by ventricular substances, it is considered to be effective and objective indicators of the diagnosis of heart failure currently. However the BNP in extreme high value (BNP>5000pg/ml), case studies reported less. The purpose of this study is to investigate the factors of affecting the prognosis of patients with heart failure in the high plasma BNP levels.Objective To investigate the factors of the prognosis affecting of patients with heart failure in the very high plasma BNP levels (BNP>5000pg/ml).Methods Retrospective analysis of69plasma BNP>5000pg/ml of patients with heart failure, divided into the group of death and group of survival, statistics separately for each group of patients with age, sex, history of hypertension, history of diabetes, history of atrial fibrillation, history of smoking, lung infection, SBP, DBP, CRP, WBC, Cr, PT, LVEF, pulmonary artery pressure and other indicators. Analysis of relevant indicators with very high plasma BNP levels in patients with heart failure prognosis.Results Comparing the two groups SBP, DBP, WBC, CRP, LVEF, pulmonary artery pressure were significantly different (P<0.05), by logistic regression analysis, DBP, WBC in patients with heart failure in the very high plasma BNP levels were independent predictors of death.Conclusion SBP, DBP, LVEF, WBC, CRP, pulmonary artery pressure were prognostic factors for death in patients with heart failure in the high plasma BNP levels, and DBP, WBC in patients with heart failure in the very high plasma BNP levels were independent predictors for death.
