| ObjectiveThe aims of our study was to analysis the relationship between systolic blood pressure at admission with the severity of impaired cardiac function and prognosis of Chinese patients with acute heart failure.MethodsThe patients(aged 29-92 years, average 69.54 years) with acute heart failure were enrolled in this study,who were were inpatient in our hospital. Cardiac function of all patients was classified as NYHAâ… toâ…£according to clinical performance. NT-proBNP,serum creatinine,hemoglobin and so on were detected.At the same time,ultrasonic cardiography was detected in these subjects,including left atrial dimension(LAD),left ventricular end-diastolic dimension (LVEDD), left ventricular end-systolic dimension(LVESD),left ventricular ejection fraction(LVEF) and so on. All patients were followed up till 6 month.ResultsThe proportion of cardiac function with class NYHAâ…£in low SBP group (A group) was significantly higher than high systolic blood pressure group (C group) and the level of lgNT-proBNP,LVEDD and LVEF among the three groups were significantly different(P<0.05).Correlation analysis also showed that systolic blood pressure level had a negative correlation with NT-proBNP,LVEDD and had a positive correlation with LVEF(P<0.01).Short term of predictive value of systolic blood pressure level in patients showed that the baseline of systolic blood pressure level were higher in event group in which cardiovascular events happened during 6 months follow-up period than in non-event group(P<0.01). Logistic regression analysis showed that LVEF, creatinine clearance rate and systolic blood pressure can assess the short-term prognosis. ConclusionsCoronary heart disease, high blood pressure are the major causes of AHF.Higher systolic blood pressure in patients were associated with better preserved cardiac function, and lower systolic blood pressure levels in patients with worse cardiac function. Except for left ventricle ejection fraction and creatinine clearance rate , systolic blood pressure levels at admission on the short-term prognosis of patients with acute heart failure also have high sensitivity and specificity. |
