The Preliminary Construction And Practice Of Quality Of Life Assessment Scale In Surgically Treated Hydatid Patients In Xinjiang Uyghur Autonomous Region

Objective:To develop Quality of Life(QOL)assessment scale in surgically treated hydatid patients,evaluation of the effectiveness of QOL assessment scale.Methods:A tentative draft was drawn based on the literature retrieval,QOL assessment in previously treated hydatid patients and with combination of expert consultation.Questionnaire was prepared with Delphi method and distributed to18experts in fields of hydatid research or relevant research. Statistic and evaluation of data were obtained in terms of the degree of activeness,coordination and authoritativeness.Using AHP to determine index weight.To analyse the scale projects by using independent sample t test,principal component analysis, eventually forming a Quality of Life Scale,and select some hospitals to apply for survery in Surgically Treated Hydatid Patients in Xinjiang,apply Mean+Standar deviation to show quality of life in patients with score,apply the method of analyzing repeated data measured to analyse Quality of Life Assessment Scale in Surgically Treated Hydatid Patients wether are different.Results:The degree of activeness in two rounds are respectively100%and94%,with degree of coordination are0.33and0.41and degree of authoritativeness are0.83and0.85.Consequently,five dimensions and30assessment indicators were confirmed. Conformance test results(CR<0.1)indicates that weight coefficient is reasonable and effective.Project analysis show that the entry of sleeping,nausea or vomiting,treatment degree of confidence,communication situation with doctors,mind degrees,moral support from friends,material support from friends,getting the opportunity about the knowledge of hydatid,the opportunity to participate in recreational activities are deleted based on judgement standard.Validity analysis results show that the scale is constituded by physiological field, psychological field, the field of independence, the field of social relationships,the environmental field,the cumulative explained variance is58.285%;Reliability analysis results show that the total scale Cronbach coefficient is 0.868,each subscale Cronbach coefficients are0.723,0.676,0.812,0.780,0.626.Analysing QOL total average score and single factors,survey results show that the fear and the friendship of indicators have not changed with the time trend(P>0.05),the remaining indicators have changed with the time trend(P<0.05);the QOL have decreased than before after one week(P<0.005),QOL have returned to the preoperative level after one month(P>0.005);Physiological field:QOL have decreased compared with the preoperative after one week(P<0.005),QOL have returned to the preoperative level after one month(P>0.005);the field of independence:QOL have decreased obviously compared with the preoperative after one week(P<0.005), QOL have decreased compared with the preoperative after one month(P<0.005),QOL have returned to the preoperative level after three months(P>0.005);The pain.the fatigue,the loss of appetite,the drug dependence,the physical strength,affecting work,the ability to work,self-care ability of QOL have decreased compared with the preoperative(P<0.005),environmental safety of QOL have improved compared with the preoperative after one week(P<0.005),the rest factors are not statistically significant compared with the preoperative(P>0.005);After one month,the pain,fatigue,loss of appetite,drug dependence of QOL have reached or exceeded the preoperative level,environmental safety of QOL have reached the preoperative level(P>0.005),the physical strength,affecting work,the ability to work,self-care ability,the family emotional support,the economic status of QOL have decreased compared with the preoperative(P<0.005);After three months, the family emotional support of QOL have decreased compared with the preoperative(P<0.005),the physical strength,affecting work,the ability to work,self-care ability,the economic status of QOL have reached or exceeded the preoperative level;After six months, the family emotional support of QOL have decreased compared with the preoperative(P<0.005),the rest factors are not statistically significant compared with the preoperative(P>0.005),or have exceeded the preoperative level(P<0.005). Conclution:QOL assessment scale system is preliminarily developed through two rounds expert questionnaire;Using AHP to determine the index weight is simple,and can enhance the accurancy of the quality of life in patients with hydatid disease surgery indicators;The scale has high reliability and validity,and can be used for quality of life in patients with hydatid disease surgery measurement;Pilot survey results show that the scale can excellently reflect the change of QOL about the preoperative or the postoperative.
